Abstract

The study aims to derive suggestion to improve the quality of care and to clarify the characteristics of care behaviors that should be intervened preferentially for good care. A questionnaire survey was conducted for 5,000 nursing home care workers across Japan, of whom a total of 1,143 respondents completed the questionnaire and were included in the data analysis. The results of the Importance-Performance Analysis (IPA) are as follows. First, support for network and dignity were included as items that should be keep up the current care behaviors. Second, high importance and low performance areas were “support to self-development of the elderly such as reading and hobbies” and “behaviors not looking into the elderly's room without permission”. Concentrated management to improve the practice of these is necessary. Third, low importance and low performance areas were included role support and autonomy support. Although the priority of the intervention is low, efforts to raise importance and performance are necessary. Finally, low importance and high performance areas were included support to maintain the health of the elderly and make a relationship of trust with elderly. The workload adjustment to these care behaviors need be reconsidered. Based on these results, we discussed the intervention for good care.

본 연구의 목적은 좋은 돌봄을 위해 우선적으로 개입해야 하는 케어행위의 특징을 규명하고 돌봄의 질을 향상시키기 위한 제언을 도출하는 것에 있다. 이를 위해 일본의 개호노인복지시설에 근무하는 케어직원 5000명을 대상으로 질문지 조사를 실시하고 결손치를 제외한 1143명을 분석에 이용하였다. 중요도-수행도 분석(IPA)의 결과, 첫째, 중요도와 수행도가 모두 높은 강점항목에 존엄지원, 교류지원 항목이 집중적으로 배치되었고, 좋은 돌봄을 위해서는 이 항목들에 대한 계속적인 관리가 필요하다. 둘째, 중요도는 높으나 수행도가 낮은 약점항목에 「이용자의 방을 허락 없이 마음대로 엿보지 않음」 「이용자가 독서나 취미활동 등 자기개발을 할 수 있도록 지원」이 해당되었고, 우선적인 개입이 필요한 항목이라 할 수 있다. 셋째, 중요도와 수행도가 모두 낮은 항목에 역할지원, 자율지원 항목들이 해당되었으며, 개입순위는 낮지만 이 항목들에 대한 중요도와 수행도를 높이기 위한 노력이 요구된다. 넷째, 중요도에 비해 수행도가 높게 나타난 항목에 노인의 건강증진 및 유지, 이용자와의 신뢰관계 형성이 해당되었으며 업무조정에 대해 제고할 필요가 있다. 이를 바탕으로 좋은 돌봄을 위한 개입책에 대해 논의하였다.

초록

본 연구는 일본 노인 입소시설인 특별양호(養護)노인홈(home)에 유니트케어(unit care)라는 새로운 시스템을 도입하면 직원과 시설환경 측면에 어떤 변화가 생기는가를 일본 오사카부(大阪府) 특별양호노인홈 케어워커 2,859명을 대상으로 실증적으로 검증하였다. 유니트케어 실시 여부에 따라 특별양호노인홈은 2집단으로, 유니트케어를 실시하고 있는 시설은 도입기간별로 3집단으로, 나누어 1) 케어워커 소진, 2) 케어업무, 3) 케어환경 등을 집단 별로 비교하였다. 연구결과 다음과 같은 사실을 알 수 있었다. 1) 케어워커의 소진은 유니트케어를 도입한 직후는 증가하지만 시간이 경과함에 따라 경감하는 경향을 보인다. 2) 기존형 시설에 비해 유니트케어형 시설에서 근무하는 케어워커가 개별케어를 더 충실히 시행함은 물론 입소자 정보도 더 잘 파악하고 있고, 서비스에 대한 자기평가 점수도 높다. 3) 기존형 시설에 비해 유니트케어형 시설에서 근무하는 케어워커는 시설운영 참가 기회도 많고, 업무상 재량권도 많이 행사하고 있으며, 동료와 상사로부터 더 많은 업무지원을 받고 있다. 이러한 결과를 통해서 유니트케어 시스템 도입이 시설 케어 질을 향상시키는 유효한 방책이라는 사실과 시스템을 빨리 정착시킬 수 있는 방안을 모색하는 것이 개호현장의 시급한 과제라는 사실도 아울러 알 수 있다.;The aim of this study was to investigate whether care workers' perceptions about care work and environment vary in relation to the number of years since the implementation of unit care systems at special nursing homes. For this purpose, we divided nursing homes into two groups, unit care facilities and non-unit care facilities, and then classified unit care facilities into three categories according to the number of years since the implementation of the unit care system. We sent our survey questionnaire to 3,919 employees of 101 special nursing homes in Osaka Prefecture and 2,859 of them responded. A notable finding of this study was that the implementation of unit care systems improved the overall quality of care work as well as care environment, even though it took several years for the unit care system to contribute to such improvements. Specifically, the negative perceptions about care (as measured with the Burnout Inventory) tended to continue for some time until the unit care system was securely established, and, started to diminish once the system was working properly. This finding leads us to conclude that the implementation of unit care systems could be an effective measure for the improvement of the overall quality of care work and care environment. For this reason, care facilities should encourage the firm establishment of the unit care system so that it can function efficiently as soon as possible.
