최근에 노인 돌봄의 필요성이 증대되면서 노인 돌봄서비스가 확충되고 있다. 그러나, 노인돌봄서비스의 전달체계에 대한 연구가 그 동안 충분히 이뤄지지 않았다. 본 연구의 목적은 노인돌봄서비스를 담당하는 공공부문의 인력(공무원과 연구자)과 민간 공급자들의 노인돌봄서비스 전달체계의 경험과 이슈를 파악, 분석하는 데 있다. 특히, 노인돌봄서비스 전달체계의 ‘통합성’을 중심으로 분석했다. 이를 위해 질적연구방법론을 채택해서 포커스그룹 인터뷰를 실시했다. 본 연구 결과에 따르면, 노인돌봄서비스 전달체계의 분절성이 심화되는 것으로 나타났다. 그 이유는 기존의 전달체계를 활용해서 서비스를 확대 제공하지 않고, 새로운 서비스를 확충하면서 각각 새로운 전달체계를 구축했기 때문인 것으로 분석됐다. 이와 함께, 기관들 간에 서비스의 연계와 조정도 원활히 이뤄지지 않고, 노인돌봄서비스의 통합적인 전산시스템도 구축되지 않은 것으로 지적됐다. 이같은 전달체계의 난맥상으로 인해 이용자의 서비스 접근성 한계, 공급자와 공공기관의 업무 증가와 비효율성 등 각종 문제가 발생하는 것으로 나타났다. 이를 개선하기 위해, 통합적 전달체계 구축, 지방정부 역할 강화, 통합전산망 구축, 예방서비스 개발 등의 대안을 모색했다.;A number of social care services for older people have been expanded recently to meet the increasing needs of that population. Despite this, there has been little research exploring the care service delivery system. The aim of this study is to examine how the public sector employees and private service providers who are in charge of the services experience and evaluate the service delivery systems, particularly in terms of the integration of services. The author chose a qualitative research method and conducted focus group interviews with a sample of both employees and providers. The research findings uncovered a fragmented service delivery system. In particular, the development of a new added service delivery system for the expanded services, rather than a modification of the existing service delivery system, created this fragmentation. In addition, there has been inadequate linkage and coordination of services in some parts. No integrated computer system for the services was established. Owing to these challenges, it has been reported the inaccessibility of services, and both public officials and private service providers have complained about the increased workloads and the inefficiency. To cope with these issues, the study suggests the establishment of an integrated service delivery system, a strengthened role of the local authority, the establishment of an integrated computer system, and the development of preventive care services for older people.
노인을 위한 대표적인 재가서비스인 가정봉사원파견사업이 노인장기요양보험의 실시등을 계기로 ‘재가노인지원서비스’로 새롭게 실시되고 있다. 그러나, 재가노인지원서비스에 대한 연구가 최근에 거의 이뤄지지 않았다. 본 연구의 목적은 경기도에 소재한 재가노인지원서비스 기관들의 서비스 제공과 기관 운영에 대한 실태 및 그 문제점과 개선방안을 탐색하는데 있다. 본 연구의 방법론은 문헌고찰과 재가노인지원서비스기관의 센터장 등 전문가들과의 ‘표적집단면접법(Focus Group Interview)’를 실시했다. 본 연구결과에 따르면, 기관들은 기존 노인돌봄서비스와의 차별성 확보, 예방서비스 제공, 안정적 인력확보 등의 측면에서 문제가 있었다. 기관의 운영 측면에서 상당수 기관들이 방문요양서비스 등 다른 사업을 동시에 겸업으로 제공하고 있었다. 그러나, 기관들은 관리와 감독이 느슨한 재가노인지원서비스는 상대적으로 소홀히 하는 반면에, 추가적인 수익이 발생하는 방문요양과 같은 겸업사업에 더 치중하는 경향이 있는 것으로 나타났다. 본 연구는 그 개선 방안으로 재가노인지원서비스의 범위 재설정, 정교한 서비스 제공 가이드라인, 기관에 대한 관리와 감독의 강화 등을 제시했다.;Along with the introduction of long-term care insurance for the elderly, the home support service for the elderly has been newly implemented, which was changed from the traditional in-home care service, home helper service in Korea. However, in recent years, there has been little research to explore the issues relating to home support service for the elderly. The aim of the study is to explore the problems and improvements for the home support service providers in Gyeonggi-do province in terms of their provision of services and management of their organizations. This research involved literature review and focus group interviews with directors of provider organizations. According to the results of the study, some service providers have limitations in competitive advantage, in provision of preventive services, and in their recruit and retainment of care workforce. In addition, there have been other limitations in terms of the management of the organizations. It seems that many service providers also have been providing other care services such as primarily home visiting care services. However, it appears that while some providers have worked harder to provide such services like home visiting service than home support service in other to make more profits. The lack of proper supervision of home support service providers by local authority appears to affect this result. In order to remove these limitations, a number of policy suggestions for the improvements of the service are recommended such as the redefinition of home support service, the developments of a number of sophisticated guidelines for the service, and the strict supervision of service providers.
