한국 전공의들의 직장 내 차별 경험과 심리적 스트레스, 우울감, 자살 생각과의 연관성
Workplace Discrimination and Its Association with Psychological Distress, Depressive Symptoms and Suicidal Ideation among Medical Residents in South Korea

Abstract

Few research has been conducted about the association between workplace discrimination and mental health outcomes among medical residents. This study sought to 1) assess the prevalence of workplace discrimination; and 2) examine the association between workplace discrimination and mental health outcomes (i.e. psychological distress, depressive symptoms, suicidal ideation) among medical residents, using a cross-sectional dataset of 1,155 Korean medical residents in 2014. Experience of workplace discrimination over the past year and its reason (i.e. gender, educational background, age, appearance, grade, and others) were assessed. Our analysis found that 27.1% (N=216) of male and 42.9% (N=153) of female medical residents experienced workplace discrimination over the past year. Among medical residents who experienced discrimination at work, the most common reasons for discrimination was educational background (32.4%) for male, whereas it was gender (69.3%) for female medical residents. Experience of workplace discrimination showed statistically significant association with psychological distress (PR: 2.27, 95% CI: 1.65-3.12), depressive symptoms (PR: 1.56, 95% CI: 1.32-1.83), and suicidal ideation (PR: 1.61, 95% CI: 1.04-2.49) among male whereas statistically significant association was observed only in the analysis of depressive symptoms (PR: 1.22, 95% CI: 1.00-1.48) among female residents. This study found that workplace discrimination was prevalent among Korean medical residents, and it was associated with mental health problems.

본 연구는 2014년에 전국 25개 전문과목 전공의 1,155명을 대상으로 수행된 단면연구 자료를 분석하여 다음의 연구목표를 달성하고자 했다. 첫째, 전공의들의 1년 동안 직장 내 차별 경험과 차별 경험의 요인은 무엇인지 확인한다. 둘째, 이와 같은 직장 내 차별 경험이 전공의들의 정신건강(심리적 스트레스, 우울감, 자살 생각)과 어떤 연관성을 가지는지 남녀로 나누어 검토한다. 분석 결과, 남성 전공의 798명 중 216명(27.1%), 여성 전공의 357명 중 153명(42.9%)이 지난 1년 동안 직장 내 차별을 경험한 것으로 보고하였다. 차별을 경험한 전공의 중 남성 전공의는 출신학교(32.4%)와 나이(27.3%), 여성 전공의는 성별(69.3%)과 출신학교(30.7%)를 차별 경험의 주된 요인으로 보고하였다. 성별을 나누어 검토한 결과, 남성 전공의에서는 직장 내 차별 경험과 심리적 스트레스(PR: 2.27, 95% CI: 1.65-3.12), 우울감(PR: 1.56, 95% CI: 1.32-1.83), 자살 생각(PR: 1.61, 95% CI:1.04-2.49)이 통계적으로 유의한 연관성을 보였다. 반면 여성 전공의에서는 직장 내 차별 경험과 우울감(PR: 1.22, 95% CI: 1.00-1.48)과의 연관성만이 통계적으로 유의한 것으로 나타났다. 본 연구는 전공의들이 경험하는 직장 내 차별이 정신 건강 문제의 원인이 될 수 있음을 제시하는 첫 연구로, 병원 내 차별 문제와 전공의의 정신건강 문제를 개선할 수 있는 정책 수립의 근거가 될 것으로 기대한다.

Abstract

The purpose of this study is to investigate the relationships between communication and psychological distress among cancer survivors and their family members. More specifically, the current study focuses on communication between family members and physicians, a major factor affecting cancer survivorship care, especially on communication within the family and the ability to communicate with physicians. The subjects are cancer survivors diagnosed with breast or colorectal cancer and one member from each of their families, and they were sampled from Seoul and Gyeonggi-do, Korea, using convenience sampling. A total of 63 cancer survivor-family dyads were sampled for analysis. As a dyadic research project focusing on the interaction between cancer survivors and family members, the current study estimated the actor-and-partner effects using the pooled regression Actor-Partner Interdependence Model (APIM), which is suitable for analyzing small-scale data sets. The findings indicated that better communication with physicians for cancer survivors was associated with lower depressive levels. For family members, better family communication was associated with lower depressive levels. The survivor-partner effect was not observed. For the family-partner effects, however, the relationship between family communication and psychological distress proved significant. That is, better family communication for cancer survivors was associated with higher anxiety and lower depressive levels for family members. The current study provided clinical and policy recommendations regarding a comprehensive approach to cancer care, psycho-social interventions for cancer survivorship care, a precise understanding of factors influencing distress, and positive communication between patients and physicians based on patient-centered care.

본 연구는 암 생존자와 가족 구성원의 의사소통이 심리적 디스트레스와 어떤 관련성이 있는지를 확인하는 것을 주요 목적으로 하고 있다. 특히, 암 생존자가 암을 관리하는 데 영향을 주는 주요 체계인 가족과 의사와의 소통에 주목하면서, 가족 내 의사소통과 의사와의 소통 능력에 초점을 두었다. 본 연구를 수행하기 위해 유방암 및 대장암 생존자와 가족 구성원 1인을 주요 대상자로 하고, 서울 및 경기 지역에서 편의표집 방법을 활용해 모집한 총 63쌍의 암 생존자-가족을 대상으로 짝 분석을 하였다. 본 연구는 행위자-상대자 상호의존 모형(Actor-Partner Interdependence Model, APIM)을 기반으로, 작은 표본 수에 적합한 ‘통합 회귀분석(pooled regression) APIM’ 기법을 사용해서 자기 효과와 상대방 효과를 추정하였다. 연구 결과, 암 생존자는 의사와의 소통 능력이 좋을 경우 우울 수준이 줄어드는 반면, 가족은 가족 의사소통이 좋을 경우 우울 수준이 줄어들었다. 암 생존자의 경우 상대방 효과는 나타나지 않았지만, 가족의 경우 암 생존자의 가족 의사소통이 가족의 디스트레스에 영향을 주는 가족의 상대방 효과가 나타났다. 즉, 암 생존자의 가족 의사소통이 좋을수록 가족 구성원의 불안 수준은 더 높은 반면, 우울 수준은 낮아졌다. 본 연구 결과는 포괄적인 측면에서의 암 생존자 접근, 지속적인 사후 관리를 위한 심리 사회적 측면에서의 개입, 디스트레스 영향 요인에 대한 명확한 이해, 환자 중심 진료에 기반한 환자-의사 간의 적극적인 소통 등에 대한 임상적, 정책적 제언을 하였다.

We examined the prevalence of self-reported suicide attempts and the relationship vetween suicide attempts and background characteristics, social integration, stress at school, psychological distress, and suicidal thoughts in a nationally representative sample of Korean adolescents. As part of the Korea Institute for Health and Social Affairs` Adolescent Health Suvey data were collected from 5,209 eighth graders and 4,677 eleventh graders from 70 junior high and senior high schools in Korea.
