의료자원의 임상적 성과를 파악할 수 있는 방법을 제안하기 위하여 본 연구는 확률변경분석과 맘퀴스트 생산성 지수를 통해 지역별 의료자원 투입이 연령 표준화 사망률에 미치는 영향을 분석하였다. 지금까지 보건 의료 부문 효율성 관련 선행연구가 의료자원을 활용하는 기관의 운영 효율성에 초점을 두었다면, 본 연구는 거시적 측면에서의 임상적 성과 효율성에 초점을 두었다. 의료자원을 효과적으로 투입하여 건강 수준을 향상시키는 것은 일종의 성과 측면의 효율성으로 이해할 수 있으며, 이는 보건 의료 부문의 재정적 지속가능성을 효과 측면에서 파악할 수 있다는 데 그 의미를 찾을 수 있다. 이에 2006~2013년, 우리나라 16개 시도를 분석의 범위로 하고, 의사 수, 전문의 수, 병상 수, 수술 수와 연령 표준화 사망률을 투입하고, 확률변경분석과 맘퀴스트 생산성 지수 분석을 수행하였다. 이때, 횡단면, 시계열 자료가 복합되어 있는 자료의 구조를 고려하기 위해 패널확률변경분석 및 맘퀴스트 생산성 지수를 각각 적용하였다. 두 분석모형 모두 의료자원의 기술적 효율성이 산출물 생산에 큰 영향을 미치는 것으로 나타났다. 본 연구에서 시도한 확률변경분석과 맘퀴스트 생산성 지수를 사용하여 지역별 의료자원의 성과 측면을 고찰하는 것은 의료자원의 최종적 목표인 임상적 측면에서의 건강 성과를 효율적으로 판단한다는 데 그 특징이 있다. 분석결과를 바탕으로 각 지역별 의료 자원의 성과적 효율성이 어떻게 나타나고 있는지 파악할 수 있으며, 공급 측면의 비용 절감을 위한 정책 방향을 제시할 수 있다는 점에서 본 연구의 의의가 있다고 할 것이다.;In order to propose a method to measure the performance of medical resources, this study empirically analyzed the impact of regional medical resource input on age standardized mortality rate through probability frontier analysis. If prior studies so far focused on the operational efficiency of institutions that use medical resources, this study focused on performance efficiency in the macro perspective. As is already well known, the probability frontier method measures the inefficiency of the analysis object in consideration of the error variation, overcoming the disadvantages of the existing data envelope analysis method. In our empirical analysis, we examined16 metropolitan cities and provinces in Korea for the period 2006-2013, and employed the panel stochastic frontier method and Malmquist Productivity Index Analysis by putting the number of doctors, the number of specialists, the number of beds, the number of operations and the age standardized mortality rate. The result showed that all the medical resources allocated to each region decreased the age-standardized mortality rate. Regional inequality in medical resources distribution should be dealt with in depth from various perspectives. In this respect, the method of applying the stochastic frontier analysis to measure the performance of regional medical resources has the advantage that the inefficiency of each input element can be examined more in depth, and this study is meaningful in this respect.
본 연구는 노인장기요양보험의 복지용구 대여서비스에 대한 조사를 통해 향후 복지용구 서비스의 정책방향에 필요한 제언을 목적으로 하였다. 조사는 서비스 이용가족을 대상으로 한 이용자 조사, 대여사업소를 대상으로 한 제공자 조사를 표적접단면접조사(FGI)를 통해 진행하였으며, 자료 분석은 질적내용분석방법(content analysis)을 사용하였다. 주요 연구결과는 다음과 같다. 첫째, 다양한 복지용구품목을 통해 질 높은 서비스 제공에 대한 필요성이 서비스 이용자와 제공자 조사에서 공통적으로 제기되었다. 둘째, 이용자 조사 결과, 서비스 계약 체결 전 단계에서 경험적으로 복지용구를 이용해 보는 것에 대한 욕구가 도출되었다. 셋째, 제공자 조사를 통해 복지용구 대여의 사업성이 낮은 점과 장기요양급여제공기록지의 보완이 시급함을 알 수 있었다. 본 연구의 결과를 토대로 향후 복지용구 대여제에 대한 방향성을 확인할 수 있었으며, 향후 전국단위의 복지용구관련 실태조사 등을 통한 후속연구가 필요할 것이다.;The purpose of this study is to analyze assistive devices rental service in the long-term care insurance. For our research purpose, we conducted focus group interviews between rental-agency and frail elders’ family. Data was analyzed using content analysis method. The main results are follows. 1) Diverse product and product item should be developed to improve the quality of assistive product from both service-users and service-providers analyses. 2) The necessity of pre-use before contract was suggested from service-users analysis. 3) The efforts to improve poor business market and the standard long-term care utilization plan is needed from service-providers survey. Our findings could suggest the policy direction of assistive product rental under the long-term care insurance system. Further research focusing on assistive product should be conducted based on national wide survey.
