본 연구는 노인장기요양보험제도에서 서비스에 대한 수급권리가 있는 1~3등급의 노인이 서비스 이용을 결정하게 되는 요인을 밝히기 위한 것이다. 특히 기존연구에서 검증된 서비스이용을 결정하는 개인적 특성 뿐 아니라 개인이 거주하는 지역적 특성이 개인의 서비스 이용결정에 어떻게 영향을 미치는가를 다층모형(HLM)을 활용하여 검증하고자 하였다. 본 연구는 Andersen의 의료이용모델을 바탕으로 노인장기요양보험제도 제2차 시범사업에서 서비스에 대한 권리가 있는 요양 1~3등급의 노인 5,989명과 9개 시범사업 지역을 연구대상으로 하였다. 종속변수는 노인들의 장기요양서비스 이용여부이며, 독립변수는 노인의 소인요인, 자원요인, 욕구요인 및 지역의 지역구분, 노인장기요양서비스의 량, 서비스의 다양성, 대체서비스 규모로 설정하였다. 분석결과 노인의 개인적 특성으로는 성, 소득수준, 수발자와의 관계, 요양욕구 및 일상생활 수행능력, 인지기능장애가 장기요양서비스 이용을 결정하는데 유의미한 요인으로 나타났으며, 지역변수로는 지역내 장기요양서비스의 다양성, 대체서비스(요양병원)의 규모, 지역구분이 유의미한 것으로 나타났다.;The aim of this study is to pin down determinants of long-term care service use of those who are eligible for the LTCI (Long Term Care Insurance). The study is based on Health Service Behavior Model of Andersen. The factors influencing the decision include such individual predisposing characteristics, enabling factors and need factors, and long term care service resources in the community. Data used here are from the 2nd pilot program for the Long Term Care Insurance scheme in which service patterns of 5,989 aged individuals and nine districts are included. Analytical method used is Hierarchical Linear Model (HLM) that can be used for the analysis of nested data structure available with current data set. The findings of the study were as follows. Women receive more benefits than men, and the proportion of those receiving care is higher in those who are eligible for National Basic Livelihood Security System (NBLSS) that in the middle income class and the low income class. In addition, the higher the family care is available, the lower the taking part in the service. Furthermore, the greater the level of difficulty with ADL functioning, the higher the abnormal behavior and the greater the care quality; and we observe higher participation rate for the scheme. Also, the higher the nursing need, the lower the service utilization we observe. In big cities and mid sized cities, more people participate in the program than in rural, and wider the available services, service utilization is higher. Also, if there exists a variety of nursing hospitals, a substitutionary entity for facility care, less people take part in the scheme in the community.
본 연구는 우리나라 노인의 거주형태의 변화와 그러한 거주형태의 변화가 갖는 의미를 심층적으로 이해하기 위하여 노인의 거주형태를 자녀와의 동거, 근거리 별거, 원거리 별거로 나누어 그 결정요인을 분석하였다. 한국 보건사회연구원에서 1998년도에 실시한 『1998년도 전국 노인생활실태 및 복지욕구조사』의 원자료를 사용한 분석결과의 주요내용은 다음과 같다. 첫째, 지속적인 핵가족화에도 불구하고 자녀가 있는 노인의 48.8%는 자녀와 동거하고 있으며 10.2%는 동일가구를 형성하고 있지는 않지만 가까운 거리에서 거주하고 있다. 둘째, 노인의 거주형태에 있어 노인의 거주지역별 차이가 매우 크다. 읍·면부 거주 노인의 경우 자녀와의 동거율은 40.4%로 동부 거주노인의 53.7%보다 낮은 반면, 원거리 별거율은 53.0%로 동부의 33.9%보다 월등히 높다. 셋째, 자녀의 거주유형에 있어 지역과 결혼상태라고 하는 배경변수, 인구학적 ‘가용성’으로서의 아들이 있는가 여부, 독립적인 소득원이 있어 경제적 ‘가능성’이 있는가, 노인이 자녀와의 별거를 선호하는가 하는 문화적 ‘선호’가 중요한 결정요인인 것으로 밝혀졌다. 따라서 예견되는 노인의 경제적 능력의 증대, 단독가구 선호의 증대, 자녀수의 감소는 미래의 노인세대들의 자녀와의 별거가구 형성률을 높일 것으로 예견된다. 위와 같은 분석결과에 기초해 볼 때 다양한 변화에도 불구하고 가족은 노인부양의 가능성을 갖고 있으므로 그 ‘가능성’을 ‘현재화’시킬 수 있는 구체적인 방안을 노인의 거주형태별로 즉, 자녀동거, 근거리 별거, 원거리 별거별로 마련하여야 할 것이다. 또한 실제적인 서비스제공 전략을 수립함에 있어서 지역적 차이를 심각히 고려해야 한다. 읍·면부의 경우 신체적인 부양을 받을 가능성이 낮은 노인이 절반을 넘고 있으므로, 이들을 위한 정책적인 관심이 필요하다.;The purpose of this study is to find factors which affect living arrangement of older parents. Previous research has dichotomized living arrangements of older parents whether they coreside with their children or otherwise. However, this study proposes that living arrangements of older parents can be better understood by dividing living arrangements of older parents in three categories such as coresidence, living in a separate household in the same community (modified extended family), living in a separate household in a distanced community (extended family). This study analyzes the data of ??Living Profile and Welfare Service Needs of Older Persons in Korea?? survey conducted by KIHASA in 1998 to understand the current status of living arrangements and to find out the determinants of living arrangements. The major findings are as follows. First, in spite of rapid nuclearization of Korean families, 48.8% of older parents coreside with their children and 10.2% are living in a separate household in the same community. Second, there is a big difference in living arrangements between rural and urban areas. In rural areas, the rate of older persons living with their children is 40.4%, while the rate in urban areas is 53.7%. On the other hand, 53.0% of older persons lived apart far away in rural areas, while the rate in urban areas is 33.9%. Third, according to the multinomial logit analysis, statistically significant determinants of living arrangement of older persons are; (1) region and marital status as background variables; (2) having a son as demographic 'availability'; (3) possession of independent income source as 'feasibility' of independent living arrangements; (4) preference to living with children as a cultural 'desirability' variable. Based on above-mentioned results, I suggest that the government must develop the policies and programmes to activate care-giving potentiality of the families by living arrangements patterns. Because more than half of older persons in rural areas live far away from their children, rural elderly are vulnerable in terms of physical care-receiving. Therefore, we have to pay attention to rural elderly in policy making and implementation of social service policies.
