The purpose of this study is to give implications to develope Korean long-term care assessment and classification instrument by analyzing foreign instrument comparatively. This study analyzed long-term care assessment and classification instruments of Japan, Germany, Untied State, Austria, United Kingdom and Singapore. Throughout object of countries, these are founded. First of all, it is different that the purpose and using of instrument is adopted in each system in each country. Some countries adopt public system for long term care(ex, Japan, Germany, and Austria), they use instrument as classification just for care needs, so their instrument is composed parsimonious items. Other countries that have not public system for long term care adopt instrument as comprehensive needs assessment about clinical function. But they have common logic throughout every countries, when they related to benefit, the instruments are developed by service need(resource utilizing) that operationalize service(care) time. Considering Points in developing Korean long-term care assessment and classification instrument are First, it should be verified reliability and validity of instrument by getting continuous and numerous datum, Second it needs to be taken a triangular position about relationship among classification of grade, need assessment for functional status and care plan. Third it needs to be decided to adequate level of efficiency and effectiveness about using compulsive or self-regulation instruments, and Forth, related to instrument, it should be discussed about long-term care assessment and classification system. Finally, Korean instrument should to being consider not only assessing need of long term care but also evaluating clinical function, care plan, and benefit.

The current paper examined political and economic environments and key actors that affected and shaped the Medicare reform through the birth of the Balanced Budget Act (BBA) of 1997. As a social product, the BBA reflects various political and economic climates as well as the interactions with key actors and interest groups. Politically, there was a consensus that Medicare would be a public program operated by the federal government and that the politics of Medicare is bipartisan, supported by Republicans and Democrats. Economically, there was a shared opinion that an economic alternative to the inefficient and overspending health care system was to achieve a market-based health system, which encourages competition between insurers. At the same time, the legislation process in the United Sates showed why incremental changes are more politically feasible than comprehensive reforms.

This study aims to evaluate the efficiency of long-term care hospitals using financial ratios, provide the information about long-term care hospitals to health care providers, and promote the activation of long-term care hospitals. We selected and surveyed 8 long-term care hospitals as the case group and 149 hospitals as the control group. But most variables about financial data in the control group were missing so we used other data about the control group, '97 hospital management analysis data conducted by Korea Institute for health and management'. We assessed the relative efficiency by comparing financial indicators of long-term care hospitals with those of other control hospitals. The main findings are as follows. First, long-term care hospitals are assessed to be higher than other control hospitals in terms of liquidity, but it is expected that long-term care hospitals are a burden on repayment of a short-term loan payable because it is inferred that they invested a short-term loan payable on fixed assets. Second, long-term care hospitals are lower than other control hospitals in terms of profitability indicators. However, it is estimated that long-term care hospitals costs less than other control hospitals, so they have potential ability in improvement of profitability indicators. Third, there is no difference in turnover ratios between long-term care hospitals and other control group. Fourth, long-term care hospitals are higher than other control hospitals in terms of growth rate indicators, but it is not sure because of data insufficiency. Fifth, long-term care hospitals are lower than other control hospitals in terms of productivity indicators, but they have higher productivity indicators in terms of inpatient performance. Sixth, long-term care hospitals are generally lower than other control hospitals in terms of utilization indicators, but they have higher inpatient utilization indicators. Seventh, long-term care hospitals are lower than other control hospitals in terms of indicators concerning employees per 100 bed and per patient. From these results, it is concluded that long-term care hospitals are not lower than other control hospitals in terms of efficiency. Furthermore, it is expected that the management efficiency of long-term care hospitals will increase due to an increase in the demand of long-term care hospital resulting from the rise in the old, chronic invalid, and rehabilitative patient.
