This study aimed to explore the caring experience and service needs of family caregivers of injured workers in outpatient treatment. In-depth interviews were conducted with 5 female spouses in their 50’s who provided care for more than 6 months to the patients with 2 to 3 grades of nursing benefits in workers’ compensation insurance. The results were drawn into 5 categories with relevant 17 sub-categories by theme analysis method. The themes were: my life is bounded by 24-hour patient care, being trapped in a dark cave, a beam of light that can withstand, getting out of the cave and into the outside world, and support required to live outside the cave. Suggestions are made based on the results of this study as follows; rehabilitation education for caregivers, vacation support system, psychological intervention program and caregiver social support group in hospital, transitional care program and social adjustment training activities for industrial accident patients, improvement of nursing benefits in workers’ compensation insurance, improving the accessibility of medical services for patients with work-related injuries.
본 연구는 산재환자 가족돌봄자의 돌봄과정의 어려움과 서비스 욕구를 파악하여 산재보험제도 내에서의 개입방안을 마련하고자 수행되었다. 의도적 표집방법을 통해 산재보험 간병등급이 2~3등급인 통원 환자의 가족돌봄자를 대상으로 일대일 심층면접을 진행하였다. 연구참여자는 척수손상으로 인한 하반신마비 환자를 돌보는 50대 여성배우자 5명이었다. 주제분석 방법을 이용하여 분석한 결과 ‘24시간 돌봄에 얽매인 나의 삶 ’, ‘캄캄한 동굴 속에 갇혀버린 나’, ‘버틸 수 있는 한줄기 빛’, ‘동굴을 빠져나와 세상 속으로’, ‘동굴 밖에서 살아가기 위해 필요한 지원’ 등 상위 범주 5개, 하위 범주 17개가 도출되었다. 본 연구결과를 바탕으로 돌봄자를 대상으로 한 재활교육 제공, 휴식‧휴가 지원 서비스 마련, 병원 내 심리정서 프로그램 마련 및 동료 돌봄자 모임 체계 구축, 퇴원전환 프로그램(transitional care program) 및 사회적응훈련을 통한 산재환자의 사회참여 활성화, 산재 간병급여의 현실화, 산재환자가 치료받을 수 있는 의료기관의 접근성 향상 등을 제언하였다.
The purpose of this study is to analyze the change of community care in the UK since it was introduced in 1990 and to explore implications for Korea. For a contextual understanding of the change, we looked at the tensions in two aspects of the system. First, we looked at the tension between the individualist element of user choice and the public element of partnership and quality of services to address the negative aspects of competition that appear behind the choice. To this end, we looked at the direct-payment scheme, partnership and service quality control, and the personal budget system. Second, we looked at the changes in the system that is responding to the environment of increasing needs and financial pressures. In this regard, we focused on the Re-ablement services introduced for prevention and recovery, changes in service eligibility standards, and emphasis on asset approach and informal care. The user choice system and the process of addressing the supply side instability behind it appear to be reflected by the formation and change of the personalisation policy, and the emphasis on asset approach and informal care in the environment of financial pressure appears to be being further reflected in the personalisation policy.
본 연구는 영국 커뮤니티 케어의 1990년 도입부터 지금까지의 긴 시간의 변화를 설명할 수 있는 프레임을 탐색하는 목적으로 수행되었다. 변화의 총체적 이해를 위하여 제도 변화에서 드러나는 두 가지 측면의 긴장을 중심으로 살펴보았다. 첫째, 이용자 선택이라는 지향과 공급자 경쟁으로 인한 부정적 측면 사이의 긴장을 살펴보았다. 이를 위해 이용자 선택 강화를 목적으로 하는 사회서비스 현금지급제도와 개인예산제도, 공급 측면의 공공성 강화를 위한 지역 파트너십과 서비스 품질관리 제도 등을 분석하였다. 둘째, 욕구증가와 재정압박의 환경에 대응해 가는 모습을 살펴보았다. 이에 대해서는 예방과 재활 목적의 단기회복 서비스 도입, 서비스 이용자격 기준 변화, 자산접근과 비공식 돌봄 강조 등을 중심으로 분석하였다. 서비스 선택제도가 가지는 양면성이 경합하는 장기간의 굴곡은 일차적으로 개인화 전략으로 귀결되면서 계속 누적되고 있는 것으로 보이며, 재정 압박의 환경에서 자산접근과 비공식 돌봄에 대한 강조가 개인화 전략에 추가적으로 투영되고 있는 것으로 나타났다.
Parents who lost their child to cancer can experience challenges in various areas of their life. Bereavement support services for these parents, however, are lacking. This study examined the bereavement service needs of bereaved mothers of children and adolescents with cancer. In-depth individual interviews were conducted with 15 bereaved mothers of a child who died before 24 years of age from cancer. Based on thematic analysis of the interviews, we found the following service needs during the end-of-life period and after the death of their child: (a) services improving end-of-life care (preparing for end of life, supporting decisions regarding hospice and palliative care) and (b) services reducing bereaved parents’ psychosocial difficulties after their child’s death (providing psychological therapy and family counseling, promoting support groups and gatherings for bereaved families, developing activities for emotional healing, improving accessible care, supporting bereaved parents’ adjustment in the community). These study findings highlight the need for bereavement support services that hospitals and community-based agencies should address to help bereaved parents handle psychosocial challenges throughout their child’s end-of-life care and after the child’s death.
소아청소년암으로 자녀를 잃은 부모들은 삶의 다양한 영역에서 어려움을 겪지만, 이들의 사별슬픔을 지원하는 서비스는 부족하다. 본 연구는 이들의 사별지원 서비스 욕구를 파악하기 위해 자녀사별 어머니 15명을 대상으로 심층 개별면담을 수행하였다. 면담내용에 대하여 주제분석을 실시한 결과, 연구 참여자들은 자녀의 죽음을 준비하는 임종 단계에서 임종 준비 지원과 호스피스 및 완화의료 이용 지원을 위한 서비스 욕구를 나타냈다. 또한, 자녀와 사별한 이후에는 사별부모들의 심리사회적 어려움을 완화시키기 위해 심리치료 및 가족상담 제공, 자녀 사별가족 모임 활성화, 정서적 힐링을 위한 활동 개발, 지속적인 상호작용과 돌봄 확대, 지역사회 적응 지원에 대한 욕구가 있음이 확인되었다. 본 연구 결과를 통해 의료기관 뿐 아니라 지역사회가 함께 자녀의 임종 단계에서부터 자녀를 사별한 이후에 겪는 사별슬픔 과정을 지원하고 이들의 심리사회적 어려움을 완화시키기 위한 사별지원 서비스가 필요함을 제언하였다.
