The purpose of this study was to identify factors related with characteristics of demographics and utilization of physical therapy service that affect satisfaction of physical therapy on elderly in senior center. The subjects were older adults aged over 60 years (total 212) and recruited in 40 senior centers located in each province. Using a survey instrument developed by the authors, subjects responded to questions concerning overall satisfaction with physical therapy services provided by senior center. The collected data was analysed with the SPSS 21.0 program. Basically, reliability checks and factor analyses method were performed twelve items reflected satisfaction of physical therapy services in senior center. Collected data was analysed frequency, chi-square test and multivariate logistic regression analysis for each satisfaction factors. Three satisfied factors including effectiveness of physical therapy, professionalism of physical therapist and availability of physical therapy department were obtained by using a factors analysis. User satisfaction with physical therapy services in senior centers was associated with the number of visits per week, the number of applied physical therapy services (hot pack, ultra sound, traction, infrared, therapeutic exercise, etc.) and subjective health condition. The importance of this study lies in its identification of factors associated with on user satisfaction with physical therapy services in senior centers.
본 연구의 목적은 노인복지관을 이용하는 노인의 인구사회학적 특성과 물리치료 이용 특성이 노인복지관 물리치료 만족도에 미치는 요인을 규명하고자 함에 있다. 연구 방법은 전국 각 시도에 위치한 40개의 노인복지관을 선정하여 각 노인복지관 물리치료실을 이용하는 이용자 230명을 대상으로 설문조사를 진행하였다. 본 연구에서는 대상자들의 인구사회학적 특성 및 노인복지관 물리치료실 이용 현황을 파악하기 위해 기술통계를 실시하였으며, 만족도 12문항에 대한 요인분석을 실시하였다. 인구사회학적 특성과 물리치료실 이용특성이 만족도에 미치는 요인 간의 관계 파악을 위해 t-test와 ANOVA를 실시하였고 만족도의 관련 요인 파악을 위해 다중회귀분석(multiple regression analysis)을 사용하였다. 요인 분석 결과 ‘물리치료 효과성’과 ‘물리치료사 전문성’, 그리고 ‘물리치료실 유효성’의 총 3개 만족도 요인이 분석되었으며, 각각의 만족도 요인들은 물리치료를 적용받는 개수가 증가할수록, 주당 물리치료를 받는 횟수가 증가할수록, 주관적 건강상태가 좋을수록 요인별 만족도가 높은 것으로 나타났다(p<0.05). 연구 결과를 바탕으로 노인이 복지관에서 이용하는 물리치료 만족도를 높이기 위한 실질적인 대안을 제시하였다.
Where are the current rehabilitation services going now and which factor is it contributing to? For this study, we used health insurance claims data (sample cohort DB, 2006~2015) to check the trends in the increase in prescribed therapeutic rehabilitation services and analyzed, by using the Log Mean Divisia Index, factors that contributed to changes in expenditure on therapeutic rehabilitation. We decomposed changes in expenditure on therapeutic rehabilitation services into demographic and medical use factors. After that, the contribution of detailed factors by treatment area was identified. Lastly, analysis has been performed by age group, types of medical service and medical care institution. The increase in therapeutic rehabilitation expenditure in the past decade was attributed more to increased number of patients and increased amount of rehabilitative therapy per patient (76.8%) than to demographic changes (23.2%). The contribution of physical therapy (79.5%) was much larger than that of non-physical therapy (20.5%). Professional therapy accounted for 57.9% of physical therapy. Those patients both less than 10 years old (4.5%) and over the age of 50 (81.9%) have been seen to contribute to the cost increase in professional physical therapy comparatively. From the characteristics of the service provider, hospitals (88.6%) or inpatient in long-term care hospitals (52.5%) has taken up most of the increase. In addition, it was founded that the use of various professional personnel is increasing along with the increase in demand for professional rehabilitation treatment (10.6%), and the costs of the pain treatment is increasing due to the aging (12.3%). Finally, it was suggested that the establishment of a rehabilitation medical delivery system and the improvement of the medical fee system are also necessary to provide integrated and specialized patient-centered rehabilitation services.
본 연구의 목적은 사회적ㆍ제도적 변화 속에서 치료적 재활서비스 영역의 의료비 증가와 주요 기여 요인을 정량적으로 파악하는 것이다. 건강보험 표본코호트2.0DB의 2006~2015년 자료와 로그 평균 디비지아 지수 방법론을 활용하여 기여도 분해 분석을 실시하였다. 치료적 재활서비스 영역의 의료비 변동을 인구학적 요인과 의료이용 요인으로 분해하여 치료 영역별 세부 요인의 기여도를 파악하였으며 연령대별, 진료 유형별, 의료기관 유형별 분석을 수행하였다. 주요 연구 결과는 다음과 같다. 지난 10년간 치료적 재활서비스 영역의 의료비 증가는 고령화 등의 인구학적 요인 변화(23.2%)보다는 질병으로 환자가 증가하거나 의료기관에서 환자 1인에게 실시하는 치료의 양이 늘어난 것(76.8%)이 주요인으로 확인되었다. 물리치료 영역의 기여도(79.5%)가 비물리치료 영역(20.5%)보다 높았고, 특히 전문물리치료 영역이 기여하는 바(57.9%)가 컸다. 전문물리치료 영역의 비용 증가를 이용자 및 공급자 특성으로 나누어 살펴보았을 때 10세 미만(4.5%)과 50대 이상(81.9%)의 인구집단과 병원급 의료기관(88.6%), 요양병원 입원(52.5%)에서 높은 기여도를 보였다. 일반물리치료 영역은 총 기여도가 21.7%에 그쳤지만, 인구 고령화와 같은 인구학적 요인의 기여도는 다른 치료 영역보다 높아(12.3%) 고령화 등으로 통증치료의 지출이 증가하고 있음을 확인하였다. 의사, 작업치료사, 사회복지사가 제공하는 비물리치료 영역의 경우 특히 환자 1인당 이용량의 변화가 높게 나타나(10.6%) 전문재활치료의 수요 증가와 함께 다양한 전문 인력의 활용이 늘어나고 있음을 확인하였다. 분석 결과를 바탕으로 치료적 재활서비스 영역 의료비 증가의 주요한 정책적 맥락을 논의하였으며 환자 중심의 통합적이고 전문화된 재활서비스를 제공하기 위해 재활의료전달체계의 정립과 수가제도의 개선이 필요함을 제언하였다.
