운동자와 비운동자간 의료이용차이의 계량적 분석

An Econometric Analysis of the Difference between Exercisers and Non-Exercisers in Medical Care Use

초록

본 연구의 목적은 의료요구의 관점에서 한국의료패널 자료를 사용하여 운동이 의료이용에 미치는 영향을 계량화하는 것이다. 즉, 운동을 하는 사람과 운동을 하지 않는 사람간에 의료이용에 차이가 존재하는지, 존재한다면 그 정도가 어느 정도인지를 probit model과 sample selection모델을 적용하여 계량화하고자 하였다. 연구결과에 의하면 운동이 의료이용에 큰 영향을 미치고 있고, 또한 운동의 유형도 의료이용에 영향을 미치는 것으로 추정되었다. 입원과 외래를 포함한 의료이용 경험률의 경우 격렬한 신체활동을 하는 사람은 운동을 전혀 하지 않은 사람에 비해 의료이용 경험률이 7.8~10.7%p까지 감소하였고, 중증도 신체활동을 하는 사람은 운동을 전혀 하지 않는 사람에 비해 8.0~12.0%p까지 의료이용 경험률이 감소하였고, 걷기 운동을 하는 사람은 6.1~8.7%p까지 감소하는 것으로 추정되었다. 또한 외래방문횟수와 입원재원일수(외래방문횟수의 3배 적용)역시 운동을 전혀 하지 않는 사람에 비해서 격렬한 신체활동을 하는 사람은 14.3~19.8%, 중증도 신체활동을 하는 사람은 15.0~22.2%, 걷기 활동을 하는 사람은 11.3~16.0% 정도 감소하는 것으로 추정되었다. 그리고 총 본인부담의료비도 운동을 전혀 하지 않는 사람에 비해서 격렬한 신체활동을 하는 사람은 13.3~19.3%, 중증도 신체활동을 하는 사람은 10.2~13.4%, 걷기 활동을 하는 사람은 9.6~14.0% 정도 감소하는 것으로 추정되었다. 본 연구의 결과는 우리나라의 자료와 연구방법의 한계를 극복하고 운동이 의료이용을 감소시킨다는 연구 결과를 뒷받침하고 있고, 또한 운동이 의료이용과 의료비 감소에 기여할 수 있는 중요한 정책수단이 될 수 있다는 점을 보여주었다고 판단된다.;This study intended to look into the effects of exercise on health care utilization using the Korea Medical Panel data adjusting for the perspective of medical needs. In other words, it intended to find out if there exist any differences in medical service uses between people who exercise and those who do not, and to quantify the differences, if any. The results of this study suggest that exercise does have great impact on health care utilization and that the type of exercise also affects the health care utilization. In the case of medical care experience estimations (in percentages) including both inpatient and outpatient care, the estimated values of strenuous exercisers were 7.8~10.7%p smaller than non-exercisers, while those of moderate exercisers fell by 8.0~12.0%p compared to non-exercisers and those of walkers went down by 6.1~8.7%p. The estimations for outpatient visits and days of inpatient stay (weighted three times higher than outpatient visits) also demonstrate similar trends. People doing vigorous physical activity were estimated to have 14.3~19.8% less physician visits and stays than non-exercisers, while those exercising moderately were to have 15.0~22.2% less and walkers, 11.3 ~16.0% less. Having overcome limitations in Korean data and research methods, the results of this study support many other research findings that conclude that exercise reduces medical care use. It is believed that they also demonstrate the potential of exercise as an important policy tool that will contribute to the reduction of medical care use and expenses. Therefore, authorities are urged to ensure that health promotion policy should be based on such research results.
