This study examined the relationship between online contact and changes in subjective health among adult game users who had experienced social isolation. It focused on the mediating role of perceptions of game-based social relationships. Previous research has primarily focused on the quantity of online activity. Less attention has been paid to the relationship between online social contact, perceived social relationships, and health. Using data from the Game User Panel Survey (2020–2024) conducted by the Korea Creative Content Agency, we analyzed 1,230 observations from 552 adult game users. All participants had experienced social isolation at least once during the study period. We conducted a bootstrap-based mediation analysis. The results showed a positive indirect effect of online contact on changes in subjective health through game-based social relationship perception. However, online contact also had a negative direct effect that was not explained by this mediator. Exploratory analyses by age group showed a significant positive indirect effect among middle-aged adults aged 45–55. No significant indirect effect was found among younger adults. These findings suggest that policies for socially isolated individuals should consider both the potential benefits and adverse aspects of online interaction. Online and offline support programs should also reflect differences in how online social relationships are connected to health across age groups.
