Abstract

This study aimed to examine the longitudinal association between post-retirement economic activity and health status and healthcare utilization among middle-aged and older adults in South Korea, conceptualizing retirement as a gradual transition process rather than a complete exit from the labor market. Using data from the 1st to 9th waves (2006–2022) of the Korean Longitudinal Study of Aging (KLoSA), we analyzed 3,065 retirees aged 50–80, comprising 12,715 observations. Participants were classified as fully retired (n=2,435) or partially retired (n=630). The study compared health status and healthcare utilization between the two retirement groups and additionally examined the association between time-varying employment status and health outcomes among partially retired individuals. Mixed Models for Repeated Measures (MMRM) and Generalized Estimating Equations (GEE) were employed to account for repeated measurements and longitudinal changes. Partially retired individuals had significantly lower odds of being diagnosed with a psychiatric disorder than fully retired individuals (OR=0.53, p=0.008). Among partially retired individuals, being employed was associated with significantly lower inpatient out-of-pocket expenditure (β=−0.12, p=0.006), lower inpatient cost per hospitalization (β=−0.06, p=0.010), and fewer outpatient visits (IRR=0.86, p=0.007) than being non-employed. These findings suggest that remaining economically active after retirement may be associated with better mental health and lower inpatient healthcare utilization among older adults. Beyond labor force participation itself, the results imply that sustaining social roles and flexible economic participation after retirement may have important implications for healthy aging and health policy in aging societies.

본 연구는 한국의 고령화 추세 속에서 은퇴를 단순한 노동시장 이탈이 아닌 점진적 전환 과정으로 정의하고, 은퇴 후 재근로 여부가 중·고령층의 건강 상태 및 의료 이용에 미치는 영향을 종단적으로 분석하였다. 고령화 연구 패널조사(KLoSA) 1~9차(2006~2022년) 자료를 활용하여 만 50~80세 은퇴자 3,065명을 분석하였으며, 완전 은퇴자(2,435명)와 부분 은퇴자(630명)의 의료 이용을 비교하는 집단 간 분석과, 부분 은퇴자 내에서 시점별 경제활동 상태가 의료 이용에 미치는 영향을 추정하는 개인 내 분석을 반복 측정 혼합모형(MMRM)과 일반화 추정 방정식(GEE)으로 수행하였다. 분석 결과, 집단 간 분석에서 완전 은퇴자 대비 부분 은퇴자의 정신과적 질환 진단 오즈가 유의하게 낮았다(오즈비=0.53, p=0.008). 개인 내 분석에서는 경제활동 상태 유지 시 비경제활동 상태 대비 입원 본인부담 비용(β=−0.12, p=0.006), 입원 1회당 비용(β=−0.06, p=0.010)이 유의하게 낮았으며, 외래진료 횟수 발생률비(IRR=0.86, p=0.007)도 유의하게 낮았다. 본 연구는 은퇴 후 경제활동 상태의 지속이 고령자의 일부 의료이용 및 정신건강 지표와 관련될 가능성이 있음을 시사하며, 이러한 결과는 고령층의 경제활동 참여를 독려하는 정책이 단순히 노동력 확보를 넘어 노년기 건강 불평등 완화와 건강한 노화를 위한 보건 정책적 근거가 될 수 있음을 보여준다.

Abstract

This study analyzes both participation in and engagement with volunteering among middle-aged and older adults in Korea using the 2022 Korean Longitudinal Study of Aging (KLoSA) and a left-censored Tobit model. Human, social, and psychological capital are examined by age group to identify life-course-specific mechanisms of resource access and conversion. Religion and social networks consistently increase both the likelihood and hours volunteered across age groups, indicating that volunteering is shaped primarily by institutional embeddedness and relational exposure. However, determinants diverge sharply by life stage. Among middle-aged adults, life satisfaction is the only significant predictor, reflecting a competitive choice structure in which volunteering competes with paid work, family responsibilities, and leisure, and thus functions as an integrative indicator of contemporaneous resource availability and conversion capacity. Among older adults, life satisfaction is unrelated to volunteering, as participation more often reflects the continuation of established social roles and institutional ties under structurally constrained choice sets. Instead, education, gender, and residential location are significant, indicating that later-life volunteering is governed by structurally accumulated resources over the life course, including organizational experience, the ability to navigate and utilize institutional resources, and community-based relational embeddedness. Overall, while volunteering constitutes the same behavioral category across age groups, participation among middle-aged adults is differentiated by short-term resource constraints within competitive life-course contexts, whereas participation among older adults is structured by accumulated capacities and embedded social positioning. These findings underscore the need for age-differentiated volunteering policies and institutional designs.

본 연구는 한국의 장년층과 노년층을 대상으로 자원봉사 참여 여부와 참여 시간을 동시에 설명하기 위해 2022년 고령화연구패널(KLoSA) 자료에 좌측 검열을 고려한 Tobit 모형을 적용하였다. 자원봉사 참여를 개인의 태도보다는 봉사 활동에 접근 가능한 구조적 위치의 문제로 재개념화하고, 인적·사회적·심리적 자본을 독립변수로 설정하여 전체 표본과 연령집단별로 비교 분석하였다. 분석 결과, 종교와 사회적 네트워크는 모든 연령집단에서 참여 가능성과 참여 시간을 유의하게 증가시켰다. 그러나 결정요인은 연령집단별로 상이하였다. 장년층에서는 삶의 만족도만이 유의하였으며, 이는 경쟁적 선택 구조 속에서 개인의 자원 가용성이 참여를 구분함을 시사한다. 반면 노년층에서는 삶의 만족도가 유의하지 않았고, 교육 수준·성별·거주 지역이 참여를 설명하였다. 이는 노년기 자원봉사가 생애 동안 축적된 구조적 자원과 지역사회 기반 관계에 의해 규정됨을 보여준다. 결과적으로 연령집단 간 차이는 심리적 요인보다 생애주기별 자원 접근 경로와 제약 구조의 차이에서 비롯되며, 이에 근거한 차별화된 정책 설계가 요구된다.
