This study was aimed at investigating the association of integrated nursing and care service program adoption with patient experience evaluation and the interaction effect of nurse staffing level. Toward this end, we performed robust regression analyses using multiple data sources including patient experience evaluation administered by health insurance review and assessment services (HIRA), adoption level of the integrated nursing and care service program, inpatient quality indicators, and hospitals’ structural and operational characteristics. Key findings are as follows. First, we observed a statistically significant positive relationships between integrated nursing and care service program adoption and physical hospital environmental domain of patient experiences evaluation and marginally significant association with overall evaluation domain, respectively. Second, we found strong evidence for the moderating effect of nurse staffing level on the association between integrated nursing and care service program adoption and patient experiences evaluation. Moderating effect of nurse staffing grade was significant in all domains of patient experience but nursing and patient rights protection. Hospitals with difficulties in retaining an appropriate level of nurse staffing may consider introducing and expanding an integrated nursing and care service program.
본 연구의 목적은 간호・간병통합서비스와 환자경험평가의 관계를 조직 수준에서 분 석하고 간호인력 확보 수준의 조절효과를 살펴보는 데 있다. 이를 위해 환자경험평가, 요양급여 적정성 평가, 의료기관 특성, 일반병동 간호등급, 간호・간병통합서비스 운영 병상 수 및 총 허가 병상 수 자료들을 이용하여 로버스트 회귀분석을 실시하였다. 주요 연구 결과는 다음과 같다. 첫째, 간호・간병통합서비스 병상 도입과 병원 환경 영역의 환자경험과 유의한 정(+)의 관계에 있었으며, 전반적 평가 영역의 환자경험과는 한계 수준에서 정(+)의 관계에 있었다. 둘째, 의료기관의 간호등급이 낮을수록 간호・간병통 합서비스 병상 도입 수준과 환자경험평가의 관계는 커지는 것으로 나타났으며, 이를 통해 간호등급은 유의한 조절변수임을 확인하였다. 이러한 간호등급의 조절효과는 간호 사 경험과 환자권리 보장 경험을 제외한 모든 영역에서 유의한 것으로 나타났다. 적정 수준의 간호인력 보유가 어려운 의료기관은 간호・간병통합서비스 병상 도입 및 확대를 통해 환자중심성 향상을 기대할 수 있을 것이다.
이 연구는 병원 조직의 의료분쟁 위기관리 커뮤니케이션 단계를 설명할 수 있는 이론적 모델을 추출하기 위한 탐색적 연구이다. 서울, 부산과 경남에 소재한 1, 2, 3차 병원의 의료분쟁 담당자 12명을 심층 인터뷰한 결과, 의료분쟁 위기관리 커뮤니케이션은 여섯 단계로 나뉘는데 ‘위기준비 단계’, ‘쟁점 관리 단계’, ‘설명과 경청 단계’, ‘협상단계’, ‘법적 대응 단계’, ‘정리 학습 단계’가 그것이다. 중요한 점은 모든 사례가 각 단계 모두를 순차적으로 거치는 것은 아니며, 사례에 따라 몇 개의 단계를 건너뛰어 진행되기도 한다는 것이다. 또 하나의 위기가 정리되면 다음 위기 준비 단계로 이어져, 전체적으로 수레바퀴 모양의 연속형 모델로 나타났다. 위기가 확대된 이후의 언론사과문을 중심으로 이루어진 기존의 위기커뮤니케이션 연구들과 달리 이 연구는 모든 위기관리 단계의 커뮤니케이션을 대상으로 분석했다. 그 결과 기존 위기관리 단계 연구들이 제시한 단일한 선형 단계와 달리, 의료분쟁 위기관리는 사례에 따라 선택적이고 연속적인 단계를 거친다는 점이 발견된 것이다. 또 실제 의료분쟁에서는 기존 위기관리 단계 모델들에서 제시된 계획 수립, 평가와 보상 등의 위기 전, 후 단계의 활동이 매우 미흡하다는 점을 발견하였다. 마지막으로 기존 위기관리 수사학 연구들에서 언급되지 않은 협상 커뮤니케이션 수사학, 즉 시간벌기, 지연, 경청, 설명, 상위권한 기법, 제 3자 개입시키기 등이 다양하게 사용되었다는 것도 이 연구의 중요한 발견점이다.;This research aims to understand the process of crisis management communication in hospital medical disputes and explore a reasonable and theoretical model for that. The data were collected from in-depth interviews with twelve medical dispute professionals at all levels of hospitals in Seoul, Busan and Kyung-nam Province. ‘The Process Model of Crisis Management Communication About Medical Dispute’ is divided into six steps, ‘Preparation for Crisis Step’, ‘Issue Management Step’, ‘Explanation Step’, ‘Negotiation Step’, ‘Legal Action Step’ and ‘Controlling and Learning Step.’ The most important point is that not every case goes through all of these steps, and some cases even skip some of them. Also, preparation for a new crisis phase tends to follow the resolution of a crisis. Most existing studies have focused on a limited number of steps centering around press reports after the crisis has been known, but this study looks at all steps in crisis management. This study finds that the crisis of medical dispute goes through selective and sequential steps that are unlike the linear process as presented in previous studies. Also, the activities in before and after crisis steps were not enough for crisis management such as planning, evaluation, and compensation. Moreover, some strategies that not mentioned in existing crisis communication rhetoric theory including ‘listening’, ‘Explanation’, ‘Contact by the Third Party’, ‘delaying action’, ‘higher authority’ have been used. That strategies can be explained as a kind of ‘Negotiation Communication Strategy’. This is one of the most significant findings of the present study.
