기온은 매개체에 의한 감염병 발생에 가장 많은 영향을 주는 기후요인으로, 말라리아는 기후변화에 민감하여 세계적으로 중요한 매개질환으로 다루어지고 있다. 본 연구는 말라리아 발생에 영향을 주는 지역적 특성을 반영하여, 기온으로 인한 말라리아 발생 위험을 추정하는 것이다. Generalized Additive Model을 이용하여, 역치기온 이상에서의 삼일열 말라리아 발생 위험을 평가하였다. 기온상승에 따라 말라리아 발생 위험이 증가하였는데, 말라리아 환자가 밀집되어 있는 서울, 경기, 인천, 강원에서 기온이 1℃ 상승하면 발생 위험이 10.8%, 12.7%, 14.2%, 20.8% 증가하였다. 위험도는 역치기온 이후에서 추정된 것으로, 역치는 지역별로 다르게 나타났다. 매개체, 가축 등 지역의 특성은 기온상승이 말라리아 발생에 영향을 미치는데 관련이 있는 것으로 보고되고 있다. 이에 따라 본 연구에서는 말라리아 발생 위험을 추정하는데 기상요소만을 포함한 경우와 지역의 특성을 함께 고려한 경우를 비교하였는데, 그 결과 모기와 가축 자료를 포함하면 모형의 적합도가 좋아졌으며, 기온으로 인한 말라리아의 위험은 낮아졌다. 앞으로 신뢰할 수 있는 정확한 데이터를 기반으로 말라리아에 영향을 미치는 여러 요인들을 효과적으로 반영한 예측 모형이 개발되어야 하겠다.;The impact of climate change on infectious diseases became one of the major public health interest in Korea. The purpose of this study is to estimate the potential effect of temperature rise on malaria transmission and to improve a prediction model by reflecting regional factors. We used claims data for health care utilization to identify malaria incidence during 2003~2010. Generalized additive model was used to examine the correlations between malaria incidences and meteorological factors. To investigate the impact of temperature on malaria incidence, we adjusted the number of Anopheles mosquitoes, the primary vector for malaria, and the animal hosts as regional factors. There were significant correlations between temperature and malaria incidence by region in Korea. For a daily mean temperature increase of 1°C above the threshold, the relative risk of malaria incidence increased except Jeju-do. When the regional factors like the number of Anopheles mosquitoes and animal hosts were added to the model, relative risks of malaria incidence were lower than those were not adjusted for regional factors in Gangwon-do, Gyeongsangbuk-do, and Chungcheongnam-do. Malaria incidence was correlated with not only meteorological factors but also regional factors in Korea. This implies that malaria incidence could be influenced by temperature rise and regional environment.

말라리아는 기후에 크게 영향을 받는 법정전염병 3군으로서 1993년 휴전선에서 재발생한 이후에 매년 환자가 발생하고 있다. 본 연구의 목적은 말라리아 발생에 있어서 기후 인자에 따른 말라리아의 발생자수 예측 모형을 구축하는데 있다. 이를 위하여 2001년부터 2011년까지의 국내의 월별 말라리아 발생사례와 평균 기온, 습도, 강우량 등의 기후인자를 수집하였다. 수집된 자료에 대하여 스펙트럼 분석을 통하여 시간지연 효과를 고찰하고, 자료 간 다중공선성을 고려한 주성분 회귀분석을 통하여 회귀모형을 구축하였다. 2009년부터 2011년 사이의 말라리아 감염병 발생자료를 통하여 검증한 결과, 구축된 회귀 모형이 말라리아 발생자료에 대한 설명력이 있는 것으로 나타났다.;Malaria is influenced by climate variable and is the third grade infectious disease of Korea Government which has been slowly increasing patient in every year since its reocurrence in the Military Demarcation Line in Korea in 1993. This study is focused on the influence analysis with climate variable and malaria occurrence and construct the model for malaria prediction based on climate factor. To do so, data of monthly malaria infection,mean temperature, relative humidity and total precipitation will be collected. To observe time lag effect, auto and cross spectral density analysis will be applied. To consider multicollinearity of each climate variable, model of malaria infection will be structed based on principal components regression method. Result shows that model explains and verifies malaria"s infectivity using infection data from 2009 to 2011.

말라리아는 우리나라에서는 1970년대 이후 사라졌지만, 1993년 휴전선 근처에서 재발생한 이후 매년 환자가 증가하고 있는 추세이다. 본 연구의 목적은 말라리아 발생에 있어서 기후요소의 영향을 살펴보고 기온 증가에 따른 말라리아 발생자수를 예측하는 것이다. 건강보험심사평가원으로부터 제공받은 2005~2007년 건강보험심사완결 원시자료 중 말라리아 16,898건이 분석대상에 포함되었다. 말라리아 발생 자료는 행정단위로 시군구, 발생기간은 주(week)를 분석단위로 하여 해당기간 시군구에서 발생한 전염병을 합산하였다. 기온자료는 전국 AWS(Automatic Weather Station)에서 3년간(2005~2007년) 최고기온, 최저기온, 평균기온, 일강수량 자료를 1일을 기준으로 작성된 자료를 이용하였다. 분석방법은 시계열 자료 분석방법인 Poisson 분포의 일반화추정방정식(Generalized Estimation Equation, GEE)을 기반으로 기온변수에 대한 3차원 스플라인, 다항 시간지연효과를 고려하였고 연간 주기성(Annual Periodicity)을 가지고 사인 함수와 코사인 함수로 표시되는 최대 6주기 조화함수인 Fourier Term을 회귀식 포함하였다. 말라리아 발생 회귀방정식에서 추정된 예측값과 기온과의 관계를 해석하는데 변곡점 영향을 살펴보았고 GIS(Geographic Information System) 상에 결과치를 표시하는 다양한 방법을 고려하였다. 분석기간 중 연도별 시군구 말라리아 발생자수는 계속 증가하였다. 2005년 평균 1.3명, 최대 262명이었으나 2006년 평균 1.6명 최대 240명, 2007년 평균 2.5명 최대 400명으로 증가하였다. 본 연구에서 제안된 기온상승에 따른 말라리아 발생예측 모형을 따라 분석을 수행한 결과 변곡점이 0.8℃, 20.2℃, 31.2℃인 영문글자 ‘M’자 모양을 나타내었다. 두 번째 변곡점(20.2℃) 이후의 경우, 기온이 1도 상승하면 주간 시군구단위 환자수가 0.212(95 CI, 0.201~0.224)명 증가할 것으로 예측되었다. 지역 결핍지수와 말라리아 발생과의 상관성을 살펴본 결과 말라리아 발생은 시군구 통합결핍지수가 낮은 지역에서 더 많이 발생하는 경향이 있었다. 말라리아 발생은 일부지역에 국한된 양상을 보였으나 해안가와 대도시의 경우 보다 세밀한 조사가 필요할 것으로 보인다.;Malaria has been eradicated since 1970’s, but has re-emerged near the DMZ in 1993. Malaria prevalence rates have tended to increase over time. The purpose of this study is to investigate the impact of weather factors on municipal malaria patient numbers, and to predict the future prevalence rate of malaria. The data came from 2005~2007 claim data from the National Health Insurance, which is calculated based on the municipal level in weekly time bands. Weather data were taken, including daily temperature and precipitation from 194 automatic weather station (AWS) managed by the Korea Meteorological Administration. The analytical approach that was used were a generalized estimation equation and a generalized linear model for a time-series of Poisson distribution. To account for the seasonal patterns of malaria not directly due to weather factors, Fourier terms with annual periodicity were introduced into the model. To allow autocorrelation due to the biological process of pathogen development and host reactions, we also considered time lags, cubic spline, and change point analysis. Malaria patients had continuously shown an increase during the study period. In 2007, for example, malaria patients were 2.5 on average and 400 in the maximum compared to those of the year 2005, which were 1.3 on average and 262 in the maximum. The distribution of the predicted model showed the shape of a character ‘M’, which had three change points in temperature 0.8℃, 20.2℃, and 31.2℃. In the prediction model of malaria, marginal temperature effects after the second change point temperature (20.2℃) was a 0.202 increase of the weekly municipal patients per 100,000 of population. While analyzing the effects of a regional socioeconomic status on malaria prevalence, we found the inverse relationship between estimated malaria patients and the deprivation index. In this study, malaria largely occurred near the DMZ, but we need more detailed investigations in specific regions such as costal areas and large cities.
