초록

본 연구는 노인장기요양보험제도 인지여부와 연관되어 있는 요인들을 검증하고, 노인장기요양보험제도의 인지여부와 65세 미만의 중 · 고령자들의 신체적 노후준비와의 관계를 고찰하는 것을 목적으로 한다. 본 연구는 도입 초기 단계의 제도에 대한 국민 인지와 이에 대한 정책적 투명성 그리고 올바른 발전 방향을 제시하는데 기여를 한다는 점에서 의미가 있다. 본 연구는 국민노후보장패널 3차 부가조사(2010)자료를 활용하여 로지스틱 회귀분석(Logistic Regression Analysis)과 일반 회귀분석(Regression Analysis) 방법을 사용하였다. 첫째, 로지스틱 회귀분석결과 학력이 높을수록, 경제활동을 할수록, 소득이 높을수록 노인장기요양보험을 인지하고 있는 것으로 나타났다. 즉 노인장기요양보험제도에 대해서 전통적인 사회복지제도의 정책대상자인 저학력층, 비경제활동인구, 저소득계층인 취약계층이 오히려 더 인지도가 낮았다. 둘째, 일반 회귀분석결과 노인장기요양보험의 인지여부와 신체적 노후준비 간 유의미한 인과관계가 있는 것으로 나타났다. 즉 노인장기요양보험을 인지하는 사람들이 노후준비를 더 잘하는 것으로 나타났다. 본 연구의 정책적 함의는 다음과 같다. 노인장기요양보험에 대해서 65세 미만의 중 · 고령자들 간에 정보의 불평등이 존재하고 있으며, 노후준비가 절실한 취약계층이 오히려 정책지식의 부족으로 제도의 혜택을 받지 못할 가능성이 크다는 점을 시사한다.;This study examines whether the awareness of middle-aged Koreans under 65 about Long-term Care Insurance. The significance of this study lies in its contribution to charting the direction for developing Long-term Care Insurance. Using the data from the Korean Retirement and Income Study (KReIS), this study utilizes logistic regression and multiple regression analysis. The results of this study can be summarized as follows. First, education, economic activity, and income are positively associated with awareness about Long-term Care Insurance. Second, awareness about Long-term Care Insurance is associated with physical preparation for people under 65. This study has an important policy implication on information equality. Although policy knowledge is associated with physical preparedness for old-age, the primary beneficiaries for social welfare who are economically inactive with a lower education and lower income, are less likely to know the existence of Long-term Care Insurance. Thus, this study suggests the importance of education on policy knowledge for policy target population.

본 연구는 한국노동패널(KLIPS) 자료를 이용해 2000년 이후 혼인(초혼)한 가구를 대상으로 혼인 이후 첫째아 출산까지의 기간을 살펴보았다. 특히 ‘친정과의 거리’가 첫째아 출산 속도와 어떠한 관련을 갖는지를 살펴보고자, 혼인가구와 친정 간의 근접도를 ①직선거리(km), ②동일 광역자치단체 소재 여부, ③범주화된 거리로 구분하여 다각적으로 분석하였다. 분석결과 여타 조건들이 동일하다면 친정과 동일한 지역에 거주하거나, 직선거리가 가까운 신혼부부가 그렇지 않은 경우에 비해 자녀를 빨리 출산하는 것으로 나타났다. 다만 친정과의 거리가 일정 수준 이상이 될 경우 거리에 따른 출산속도의 차이가 비례적으로 나타나지는 않았다. 본 연구는 대표성 있는 통계자료를 이용해 저출산 문제가 심각한 사회문제가 된 2000년대 들어 혼인한 가구를 대상으로 친정과의 거리와 첫째아 출산 속도 간의 관련성을 실증적으로 살펴보았다. 또한 한국사회의 특성에 대한 이해를 바탕으로 (외)조부모 돌봄이 갖는 의미를 살펴보고, 이를 통해 저출산 문제해결을 위한 방안의 하나로 (외)조부모의 돌봄에 대한 정책발굴의 필요성을 제시하였다.;This study used data from the Korea Labor and Income Panel Study (KLIPS) to examine how newly married couple's residential distance to the wife’s parents affects the timing of childbirth.. In particular, we closely examined the period between marriage and first childbirths for the households married since 2000. We measured the newly married couple’s residence distance to wife’s parents house in 3 ways: proxy distance measured by km (logged for analysis), categorized distance to 5 groups, and residence in the same macro district. The key findings can be summarized as follows: First, in general, the newly married couples who live in a same macro district with their wife’s parents are likely to have a child earlier than those who do not. Second, the residential distance (measured by logged km) between a newly married couple and the wife’s parents is positively related to having a first baby earlier. However, the analysis with the categorical measure of residence distance reports that the effect of residence distance is not linear in that the two longest distance groups were not statistically different in terms of childbirth probability compared to the reference group which has the shortest distance. Despite several limitations in the study sample, the measurement, and the possibility of residence movement, this study may help build knowledge regarding newly married couple’s behaviors of childbirth. This study concludes with several policy implications for boosting childbirth among newly married couples.

본 연구는 1999년 장기이식을 전제로 뇌사를 법적으로 인정하는 ‘장기등이식에 관한법률’이 제정되어 장기이식을 위한 법적 기반은 마련되었으나, 법 제정 이후 뇌사자의 장기기증은 오히려 감소되고 있고 있는 시점에서 일반국민의 장기기증에 대한 태도와 이에 영향을 미치는 요인을 분석하여, 그 결과를 바탕으로 향후 장기기증 활성화를 위한 기초 자료를 제공하는 것을 목적으로 하였다. 본 연구는 한국보건사회연구원에서 실시된 ‘뇌사 및 장기이식에 대한 여론조사’ 자료를 이용하였으며, 본 연구의 주요 결과는 다음과 같다. 첫째, 뇌사의 개념에 대한 지식 정도, 뇌사를 사망으로 받아들이는 뇌사 수용도, 조사대상자 본인의 장기기증 의사, 조사대상자 가족의 장기기증 의사에서 성별, 연령, 결혼상태, 교육수준, 직업, 월평균 가구소득 등 인구학적·사회경제적 변수에 따른 집단간 차이를 나타내고 있었다. 둘째, 경제적 능력에 따른 장기이식수술 의사의 경우, 장기이식 수술 의사가 있음을 응답한 사람의 대부분은 장기이식 수술비용 부담 능력이 없었다. 셋째, 조사대상자 본인 및 조사대상자 가족의 장기기증 의사에 대한 로지스틱 회귀분석(logistic regression) 결과, 조사대상자 본인의 장기기증 의사에 영향을 미치는 변수로는 결혼상태, 가족의 장기기증 의사, 사후 사체훼손에 대한 거부감, 장기이식술에 대한 인식으로 나타났고, 조사대상자 가족이 뇌사상태일 경우 가족의 장기기증 의사에 영향을 미치는 요인으로는 가구소득, 뇌사에 대한 지식, 뇌사 수용도, 장기이식술의 필요성, 본인의 장기기증 의사로 나타났다. 우리나라는 뇌사인정에 대한 공감대가 형성되지 않고 있고, 전통적인 유교사상의 영향, 사체훼손에 대한 두려움, 가족간의 장기기증에 대해서는 관대하나 타인에 대한 장기기증에 소극적인 가족 중심적인 사고방식 등 우리나라 고유의 문화적·윤리적 영향을 많이 받고 있는 것으로 생각된다. 때문에 우리나라에서 장기기증을 활성화시키기 위해서는 장기기증에 대한 일반인들의 이해 부족을 줄여 사회적으로 공감대를 형성하기 위한 노력을 해나가야 할 것이며, 특히 우리나라의 고유의 문화적·윤리적 환경 속에서 적절한 장기기증 활성화 방안을 찾아야 할 것이다.;This study aims to analyze public attitudes toward organ donation and several relevant factors in a current situation where, despite the 1999 legislation of Organ Transplantation Law, the number of brain-dead donors is decreasing. This study is based on  “A Survey on Public Opinion on Brain Death and Organ Transplantation” conducted in 2002 by the Korea Institute for Health and Social Affairs. The subject group consists of 1,016 people who have been selected through systematic sampling from a group of 4,000 adult men and women over the age 20 by trained telephone survey workers between July 8, 2002 to July 15, 2002. This study utilizes SAS for data analysis and chi-square test and logistic regression analysis to understand what effects demographic and socioeconomic factors have on peoples attitudes toward organ donation. Some of the main findings of this study are as follows. First, there were differences in people’s knowledge of the concept of brain death, acceptance of brain death, the respondent’s intention of organ donation, and the respondent's intention of family members’ organ donation between demographic and socioeconomic groups. Second, as to the respondent’s-to have an organ transplant operation, most of those who responded as having willingness to receive an organ donation were those without the ability to pay for the cost of organ transplant operation. Third, affecting variables to the respondent's intention of organ donation were marital status, respondent's intention of family members’ organ donation, traditional view of human body, awareness of organ transplantation. Affecting variables to the respondent's intention of family members' organ donation were monthly income level, knowledge of the concept of brain death, reluctance to accept brain death, awareness of organ transplantation, and the respondent's intention of organ donation. There are some cultural differences between Korea and West, which may explain the reluctance to donate organs. So, it is not enough that the system and technology of organ transplantation have been imported, a cultural and ethical background transplant ins also needed in Korea. From these results, this study draws several policy suggestions. First, as seen from the case of such developed countries as Spain and the US, there is a need for government-initiated publicity and education efforts to change people’s attitudes toward brain death and organ transplantation. Second, there is a need for institutional means to prioritize the individual’s consent to organ donation over his or her family members refusal of it. Also, the government should make effort to reduce the patient’s cost of organ transplantation and thus to achieve national equity in the distribution of organs.
