한 국가의 건강보장제도 평가 기준인 의료서비스이용 현황을 살펴보기 위해 의료필요가 있음에도 불구하고 사회경제적 요인으로 인해 적절한 의료이용을 하지 못하는 미 충족의료 현황을 분석하고 관련요인을 분석하는 것은 중요하다. 이 연구에서는 1차, 2차 한국복지패널 자료를 이용하여 경제적 이유로 인한 미 충족의료 현황과 관련요인을 분석하였다. 우선 연도별 단면적 기술 분석과 관련요인 분석(로지스틱 회귀분석)을 수행하고 2개년도 자료를 연동하여 동태적 기술 분석과 관련요인 분석(로지스틱 회귀분석)을 수행하였다. 단면적 분석결과 1차, 2차 한국복지패널자료의 미 충족의료 유경험 빈도는 각각 5.54%, 2.99%이었고 미 충족의료 유경험에 관련되는 요인으로 가구주 연령, 교육, 주관적 건강상태와 가구 소득계층이 유의하였는데 특히 가구 소득계층과 가구주 주관적 건강상태가 가장 유의하였다(p<0.05). 동태적 분석결과 2년 동안 미 충족의료를 한번 이상 경험할 가능성, 충족군에서 미충족군으로 추락할 가능성, 미충족군에서 미충족군으로 계속 잔류할 가능성에 관련되는 요인으로 가구 소득계층과 가구주 주관적 건강상태가 유의하였다(p<0.05). 분석 및 자료의 한계점에도 불구하고 이러한 분석 결과는 정책적으로 미 충족의료를 개선하기 위해 충분한 소득 지원과 건강수준을 보호, 증진하는 것이 여전히 중요함을 시사한다.;A study on unmet medical need is instrumental to understanding the condition of medical utilization as a measure for the evaluation of a national health care system. Using the 1st and 2nd waves of Korea Welfare Panel Data, the prevalence and association factors of unmet medical need due to economic difficulty were analysed. For this ends, the cross-sectional analysis and the longitudinal analysis with descriptive and analytic step were implemented. The prevalence rates of unmet medical need were 5.54% (1st wave), 2.99% (2nd wave) and the significant factors associated with unmet medical need were householder's age, householder's education level, householder's subjective health status, and house income class. The factors strongly associated with the probability of their having experienced unmet medical need, or having been shifted from the group with a met medical need to the one with an unmet medical need, or having been left in the group with an unmet need, were house income class and householder's subjective health status. In spite of the limitations of analysis and data, these results suggest that policies for ensuring the adequate income security, the protection of health, and the promotion of health are still important for the poor to escape from the unmet medical need status.

본 연구는 취약계층인 독거노인의 미충족 의료 현황과 영향요인을 분석하기 위해 수행되었다. 이를 위해 2014년 노인실태조사 자료를 이용하였으며, Gelberg 등(2000)의 취약계층 행동모델을 기반으로 위계적 로지스틱 회귀분석을 실시하였다. 분석결과, 독거노인은 생활전반에 걸쳐 취약한 상태이며, 이들 중 25.7%가 미충족 의료를 경험한 것으로 나타났다. 소인요인 중 여성, 전기노인, 취업상태, 낮은 교육수준, 비자발적 독거가 미충족 의료의 영향요인으로 나타났으며, 가능요인 중 낮은 가구소득, 의료기관까지 짧은 소요시간, 낮은 자녀와의 관계 만족도, 적은 지인 수가 영향요인으로 나타났다. 필요요인 중에서는 많은 만성질환 개수, 기능제한, 우울증, 나쁜 영양상태가 미충족 의료의 주요 영향요인으로 나타났다. 본 연구결과는 취약한 독거노인에 대한 사회적 관심의 필요성과 이들의 의료접근성 향상을 위한 충분한 소득지원, 부족한 사회적 지지에 대한 보완, 독거노인의 취약특성을 중심으로 한 건강관리 지원의 중요함을 시사한다.;The purpose of this study was to examine the current status and predicting factors of the unmet needs of the elderly living alone in Korea. For this study, we analysed data from the 2014 National Survey on Older Koreans. Hierarchical logistic regression analysis was used to examine influencing factors based on the Behavioral Model for Vulnerable Populations. According to the results of this study, the elderly living alone are vulnerable throughout their entire life course. 25.7% of the respondents were shown to have unmet medical needs. The factors that predicted unmet medical needs were age, employment status, low education level, involuntary solitary living, low household income, distance to medical centers, low relationship satisfaction with their children, lack of close friends, number of chronic diseases, limitations in daily functions, depression, and nutritional status. These results suggest urgent needs for policies to provide adequate income security, social support mechanisms, and access to necessary health care services to fill the needs of vulnerable elderly living alone.

본 연구는 의사결정트리를 이용하여 성인의 미충족 치과의료 경험의 영향 요인을 파악하고, 미충족 치과의료 경험 가능성이 높은 위험집단 및 치과의료 미충족 이유에 따른 집단을 분류하여 그 특성을 분석하였다. 자료는 제6기(2013~2015년) 국민건강영양조사의 만 19세 이상 성인 15,918명이며, 연구에 이용된 변수는 인구사회학적 특성, 건강행태적 특성, 정신인지적 특성, 건강관련 특성, 구강건강 특성 등이다. 자료는 복합표본분석, 의사결정트리, 로지스틱회귀 분석을 실시하였다. 연구결과 미충족 치과의료에 미치는 영향 요인은 저작불편호소가 가장 중요한 요인이었으며 다음으로 가구소득수준, 스트레스, 연령, 경제활동 상태, 고할압 의사진단 등의 순으로 나타났다. 저작불편을 호소하고 가구소득 수준이 낮은 65세 이상의 보험급여 가입자 그룹의 미충족 치과의료 경험이 67.8%로 전체 미충족 치과의료 경험률 31.3% 대비 두 배 이상 높았다. 또한 미충족 치과의료의 이유를 의료접근성 모형의 단계에 따라 위험집단의 특성을 파악해 본 결과, 의료필요의 인지(12.5%), 의료추구(21.8%), 의료도달(27.0%), 의료이용(36.9%)의 단계에 이를수록 미충족 치과의료의 이유에 대한 분율이 높았다. 본 연구는 미충족 치과의료의 영향요인과 이유를 체계적으로 분석하기 위해 의료접근성 모형 및 의사결정트리를 이용하여 위험집단을 분류하고 이들의 특성을 파악하였다. 치과의료 접근성 향상을 위해서는 미충족 치과의료 위험집단의 특성별로 의료인지, 의료추구 단계에서부터 보건교육, 사회적 지지 등의 개입이 필요하다.;The purpose of this study was to analyze the factors affecting the unmet dental care of adults using decision trees and to classify them according to the risk group of unmet dental care and the group of reasons for unmet dental care. The data are 15,918 adults aged 19 and over in the National Health and Nutrition Examination Survey during the 6th period (2013 - 2015). The variables used in the study were socio-demographic characteristics, health behavior characteristics, mental health characteristics, oral health characteristics. Data were analyzed by composite sample, decision tree, and logistic regression analysis. The results of this study are as follows. Dental complaints such as mastication discomfort were the most important factors affecting unmet dental care, followed by household income level, stress, age, economic activity status. 67.8% of those who have a complaint of mastication discomfort, low income and medical aid-insured group 65 years old or older experienced unmet dental care. This is over two times compared with overall unmet dental care experience 31.3%. In addition, the reason for the unmet dental care was classified according to the stages of the access model to health care. The results were as follows: recognition of dental needs (12.5%), dental care pursuit (21.8%), dental reach (27.0%), and use of dental care (36.9%). This study analyzed systematically the factors and reasons of unmet dental care using a health care access model and decision tree analysis. In order to improve accessibility of dental care, interventions such as health education and social support are needed for each type of unmet dental health risk group, especially classified according to early stages of access to health care such as first stage of perception of needs and desire for care and second stage of health care seeking.

본 연구는 오늘날 우리 사회의 고령화 속도와 더불어 증가하고 있는 노인들의 자살과 관련하여 자살시도 전 단계인 자살 생각에 영향을 미치는 요인을 탐색하고, 관리 방안을 분석하기 위해 수행되었다. 이를 위해 노인의 자살 생각에 영향을 미치는 유발변인을 개인체계, 가족체계, 사회체계로 구분하여 생태체계적 관점에서 살펴보았다. 연구대상은 2011년도 국민건강영양조사 자료의 65세 이상 노인남녀 1,447명을 대상으로 복합표본 로지스틱회귀분석을 실시하였다. 연구결과 우리나라 노인의 자살 생각 유발변인으로는 개인수준에서는 연령(OR=1.05, 95% CI=1.02-1.08), 우울(OR=8.40, 95% CI=0.08-0.19), 주관적 정신건강(OR=6.79, 95% CI=0.62-5.01)이 노인 자살 생각에 영향을 미치는 변수로 나타났으며, 사회수준에서는 필요 의료서비스 미충족 여부(OR=1.52, 95% CI=1.01-2.30)가 노인 자살 생각에 영향을 미치는 변수로 나타났다. 본 연구결과, 우리나라 노인 자살을 예방하기 위한 정책의 방향이 개인적 차원에서 벗어나 개인, 가족, 사회 환경을 모두 고려한 다 차원적 접근이 중요함을 알 수 있었다.;In the aging society, elderly suicide rates in Korea are higher than that of any other age group. The design of effective suicide prevention could start from identifying factors for elderly suicide ideation. Although suicide ideation is influenced by many factors, most studies have focused on individual factors alone. Thus, this study investigated factors related to the Korean elderly suicide ideation using eco-systematic analysis including individual, family, and social levels. Study subjects include 1,447 adults older than 65 using the data from 2011 National Health and Nutrition Survey. Data were analyzed using frequency test to show descriptive statistics and complex samples logistic regression considering the stratified, clustered, and systematic sampling design of National Health Nutrition Survey. The results of complex samples logistic regression showed that age, depression, and self-rated health at the individual level and unmet need of medical need at the social level were significant factors of suicide ideation. The findings of this study revealed that it is critical to establish appropriate intervention system and programs considering all three levels of eco-systematic analysis to prevent elderly suicide ideation.
