Abstract

This study aimed to examine the longitudinal association between post-retirement economic activity and health status and healthcare utilization among middle-aged and older adults in South Korea, conceptualizing retirement as a gradual transition process rather than a complete exit from the labor market. Using data from the 1st to 9th waves (2006–2022) of the Korean Longitudinal Study of Aging (KLoSA), we analyzed 3,065 retirees aged 50–80, comprising 12,715 observations. Participants were classified as fully retired (n=2,435) or partially retired (n=630). The study compared health status and healthcare utilization between the two retirement groups and additionally examined the association between time-varying employment status and health outcomes among partially retired individuals. Mixed Models for Repeated Measures (MMRM) and Generalized Estimating Equations (GEE) were employed to account for repeated measurements and longitudinal changes. Partially retired individuals had significantly lower odds of being diagnosed with a psychiatric disorder than fully retired individuals (OR=0.53, p=0.008). Among partially retired individuals, being employed was associated with significantly lower inpatient out-of-pocket expenditure (β=−0.12, p=0.006), lower inpatient cost per hospitalization (β=−0.06, p=0.010), and fewer outpatient visits (IRR=0.86, p=0.007) than being non-employed. These findings suggest that remaining economically active after retirement may be associated with better mental health and lower inpatient healthcare utilization among older adults. Beyond labor force participation itself, the results imply that sustaining social roles and flexible economic participation after retirement may have important implications for healthy aging and health policy in aging societies.

본 연구는 한국의 고령화 추세 속에서 은퇴를 단순한 노동시장 이탈이 아닌 점진적 전환 과정으로 정의하고, 은퇴 후 재근로 여부가 중·고령층의 건강 상태 및 의료 이용에 미치는 영향을 종단적으로 분석하였다. 고령화 연구 패널조사(KLoSA) 1~9차(2006~2022년) 자료를 활용하여 만 50~80세 은퇴자 3,065명을 분석하였으며, 완전 은퇴자(2,435명)와 부분 은퇴자(630명)의 의료 이용을 비교하는 집단 간 분석과, 부분 은퇴자 내에서 시점별 경제활동 상태가 의료 이용에 미치는 영향을 추정하는 개인 내 분석을 반복 측정 혼합모형(MMRM)과 일반화 추정 방정식(GEE)으로 수행하였다. 분석 결과, 집단 간 분석에서 완전 은퇴자 대비 부분 은퇴자의 정신과적 질환 진단 오즈가 유의하게 낮았다(오즈비=0.53, p=0.008). 개인 내 분석에서는 경제활동 상태 유지 시 비경제활동 상태 대비 입원 본인부담 비용(β=−0.12, p=0.006), 입원 1회당 비용(β=−0.06, p=0.010)이 유의하게 낮았으며, 외래진료 횟수 발생률비(IRR=0.86, p=0.007)도 유의하게 낮았다. 본 연구는 은퇴 후 경제활동 상태의 지속이 고령자의 일부 의료이용 및 정신건강 지표와 관련될 가능성이 있음을 시사하며, 이러한 결과는 고령층의 경제활동 참여를 독려하는 정책이 단순히 노동력 확보를 넘어 노년기 건강 불평등 완화와 건강한 노화를 위한 보건 정책적 근거가 될 수 있음을 보여준다.

본 연구의 목적은 지역사회에서 수행하고 있는 보건사업이 폭염으로 인한 열성질환의 위험을 완화하는 데 효과적으로 기여하는가를 평가하는 것이다. 이를 위하여 폭염 대응 보건사업의 수행 정도에 따라 열성질환의 위험과 적응 수준이 다르게 나타나는지 비교하였다. 먼저 보건사업의 정량적 수행 정도를 측정하기 위하여 지표를 개발하여 적용하였으며, 정성적 수행 정도를 측정하기 위하여 사업담당자의 자가평가를 활용하여 점수를 산출하였다. 다음으로 폭염으로 인한 열성질환의 위험도는 일반화 부가모형(Generalized additive model, GAM)을 활용하였다. 최종적으로 폭염 대응 보건사업의 효과를 평가하기 위하여 보건사업 수행정도에 따라 기온증가로 인한 외래 의료이용의 위험이 증가하기 시작하는 시점의 기온과 기준 기온 이후에서의 열성질환의 발생위험을 비교하였다. 연구 결과, 사업 수행 정도가 높은 지역들은 역치기온이 높게 나타나, 보건사업이 폭염의 직접적인 건강영향인 열성질환에 대한 적응역량을 강화시키는 데 기여하고 있는 것으로 보인다. 그러나 고온에서는 사업 수행 정도와 관계없이 모든 지역에서 위험도가 급격히 증가하여, 고온에서 일어나는 위험을 완화하기 위한 추가적인 대책이 필요하다.;This research aims to evaluate the performance of community public health interventions, to explore linkages between high temperature and health risks and ultimately to identify the effectiveness of community public health interventions in alleviating impacts of high temperatures on human health. Three separate analyses were performed for the research goals. Firstly, we developed indexes and criterions for quantitatively evaluating community public health interventions, and also evaluated the qualitative performance of the intervention programs using the self-evaluation of those who were in charge. Secondly, we estimated the association between high temperatures and health care utilization by using generalized additive models (GAMs) with non-parametric smoothing functions to describe nonlinear relations. Thirdly, the effectiveness of community public health interventions was evaluated by threshold temperature and RR for the association between high temperatures and health care utilization. The results suggested that high temperatures were associated with increased health care utilization among patients with heat-related illness, and generally nonlinear relations (J shaped) were observed. For heat-related illness, the estimated thresholds were on average high in communities among the high performance group. The high thresholds observed for the high performance communities might be attributed to an improved adaptation of local residents to heat, thanks to the public health interventions. However, there is a need to implement adaptation actions for focusing on vulnerable groups.

기온은 매개체에 의한 감염병 발생에 가장 많은 영향을 주는 기후요인으로, 말라리아는 기후변화에 민감하여 세계적으로 중요한 매개질환으로 다루어지고 있다. 본 연구는 말라리아 발생에 영향을 주는 지역적 특성을 반영하여, 기온으로 인한 말라리아 발생 위험을 추정하는 것이다. Generalized Additive Model을 이용하여, 역치기온 이상에서의 삼일열 말라리아 발생 위험을 평가하였다. 기온상승에 따라 말라리아 발생 위험이 증가하였는데, 말라리아 환자가 밀집되어 있는 서울, 경기, 인천, 강원에서 기온이 1℃ 상승하면 발생 위험이 10.8%, 12.7%, 14.2%, 20.8% 증가하였다. 위험도는 역치기온 이후에서 추정된 것으로, 역치는 지역별로 다르게 나타났다. 매개체, 가축 등 지역의 특성은 기온상승이 말라리아 발생에 영향을 미치는데 관련이 있는 것으로 보고되고 있다. 이에 따라 본 연구에서는 말라리아 발생 위험을 추정하는데 기상요소만을 포함한 경우와 지역의 특성을 함께 고려한 경우를 비교하였는데, 그 결과 모기와 가축 자료를 포함하면 모형의 적합도가 좋아졌으며, 기온으로 인한 말라리아의 위험은 낮아졌다. 앞으로 신뢰할 수 있는 정확한 데이터를 기반으로 말라리아에 영향을 미치는 여러 요인들을 효과적으로 반영한 예측 모형이 개발되어야 하겠다.;The impact of climate change on infectious diseases became one of the major public health interest in Korea. The purpose of this study is to estimate the potential effect of temperature rise on malaria transmission and to improve a prediction model by reflecting regional factors. We used claims data for health care utilization to identify malaria incidence during 2003~2010. Generalized additive model was used to examine the correlations between malaria incidences and meteorological factors. To investigate the impact of temperature on malaria incidence, we adjusted the number of Anopheles mosquitoes, the primary vector for malaria, and the animal hosts as regional factors. There were significant correlations between temperature and malaria incidence by region in Korea. For a daily mean temperature increase of 1°C above the threshold, the relative risk of malaria incidence increased except Jeju-do. When the regional factors like the number of Anopheles mosquitoes and animal hosts were added to the model, relative risks of malaria incidence were lower than those were not adjusted for regional factors in Gangwon-do, Gyeongsangbuk-do, and Chungcheongnam-do. Malaria incidence was correlated with not only meteorological factors but also regional factors in Korea. This implies that malaria incidence could be influenced by temperature rise and regional environment.
