This study aims to provide an in-depth analysis of the motivations for establishing and participating in cooperatives among cancer survivors using qualitative research methods. The findings reveal that the motivations for forming these cooperatives extend beyond simple economic necessity, encompassing multidimensional factors such as addressing institutional gaps, ensuring psychological stability, and maintaining continuity in lifestyle management. In particular, participants faced institutional limitations within the existing healthcare system, including financial burdens during periods without insurance coverage, constraints on accessing long-term care, and rising costs of non-reimbursable treatments. Consequently, they expected the co-operative to function as an alternative social safety net to bridge these gaps. These co-operatives offer practical strategies for comprehensively overcoming economic, social, and psychological challenges through mutual support networks and low-cost long-term care and lifestyle management services funded by contributions and membership fees. Furthermore, participants transcended the role of passive beneficiaries, gaining agency as active service providers and decision-makers within the co-operative. This study is distinguished from existing literature by its focus on the unique nature of co-operative participation among cancer survivors and the potential of their proactive solidarity. It suggests the possibility of a shift in the healthcare paradigm away from a curative-centered healthcare system toward a community-based model focused on prevention and daily lifestyle practices.
본 연구는 암 경험자의 협동조합 설립 계기와 참여 동기를 질적 연구 방법으로 심층 분석하는 데 목적이 있다. 분석 결과, 협동조합 설립 동기는 단순한 경제적 필요를 넘어 제도적 한계 보완, 심리적 안정 확보, 생활습관 관리 지속이라는 다차원적 요인으로 도출되었다. 암 경험자들은 보험사가 보험금 지급 책임을 지지 않는 일정 기간 치료비, 장기 요양 접근 제약, 비급여 치료비 부담 등의 기존 보건의료 체계의 제도적 한계에 직면하고 있었으며, 협동조합이 이를 보완하는 대안적 사회 안전망으로서 기능하기를 기대하고 있었다. 협동조합은 출자금 및 회비를 기반으로 한 저비용 장기 요양·생활관리 서비스와 상호 지지 네트워크를 통해 경제적·사회적·심리적 어려움을 통합적으로 극복하기 위한 실천 전략을 제시한다. 참여자들은 협동조합을 통해 수혜자를 넘어 서비스 제공자 및 의사결정자로서 주체성을 경험한다. 본 연구는 암이라는 특정 질병 경험자들의 협동조합 참여라는 특수성과 암 경험자의 주체적 연대가 지닌 잠재력에 주목했다는 점에서 기존 연구와 차별성이 있다. 기존의 사후 치료 중심 의료 체계에서 벗어나 사전 예방 및 생활 실천 중심의 공동체 모델의 실현을 통한 보건의료 패러다임의 전환 가능성을 시사하였다.
This study applies the Andersen Behavioral Model to data from the 2023 Survey of Older Koreans to examine the determinants of willingness to use six paid home care services supporting Aging in Place (AIP). Since the survey asked about payment intention only among those who reported needing a given service, this study examines the stage at which perceived need is converted into willingness to bear the cost, rather than service use itself. Whereas previous applications of the model have primarily addressed whether publicly funded services are used, for which eligibility is determined by assessed functional need, this study analyzes older adults' own willingness to bear the cost of private services, where no such screening exists. Applying an identical specification across six services further confirms that the observed factor structure is not confined to any single service. Education level, subjective economic status, and opportunities for neighborhood interaction were significant positive predictors across all six services, whereas need factors were significant for only some services and varied in direction. The formation of payment intention after need recognition is thus explained more consistently by individual resources than by functional status, indicating that older adults with limited resources are less likely to secure services through the private market. The study proposes dual access pathways within the same service according to ability to pay, together with support for establishing service supply capacity and public provision of service information.
본 연구는 2023년 노인실태조사를 바탕으로 앤더슨 행동모델을 적용하여, 6개 재가서비스(주거환경 개선, 일상생활 지원, 안전 지원, 방문 의료·건강 서비스, 병·의원 동행 및 외출 지원, 상담 서비스)의 유료 이용의향 결정요인을 분석하였다. 노인실태조사는 각 서비스가 필요하다고 응답한 노인에 한하여 본인 부담 이용 의향을 질문하므로, 본 연구는 서비스의 필요가 인식된 이후 그것이 이용 의사로 전환되는 국면을 분석 대상으로 삼는다. 기존 앤더슨 모델 적용 연구들이 주로 기능 상태에 따라 수급 자격을 심사하는 공적 급여의 이용 여부를 다루어 온 것과 달리, 본 연구는 자격 심사가 존재하지 않는 민간 서비스 영역을 대상으로, 노인 당사자의 본인 부담 의사를 분석함으로써 모델의 적용 범위를 확장하였다. 또한 6개 서비스에 동일한 모형을 반복 적용하여 요인 구조가 특정 서비스에 국한된 현상이 아님을 확인하였다. 분석 결과, 교육수준과 주관적 경제상태, 이웃교류기회는 6개 서비스 전부에서 유의한 정적 영향을 보인 반면, 욕구요인은 일부 서비스에서만 유의하였고 영향의 방향도 서비스에 따라 상이하였다. 즉 필요 인식 이후의 지불 의사 형성은 기능 상태보다 개인이 보유한 자원에 의해 더 일관되게 설명되었다. 이는 자원 수준이 낮은 노인이 민간 시장을 통해 서비스를 확보하기 어려움을 의미하는바, 이를 토대로 동일 서비스 내에서 지불 능력에 따라 접근 경로를 이원화하는 설계와 민간 서비스의 공급 기반 조성 및 공적 정보 제공을 제언하였다.
