Abstract

In Korea, the demand for medical care for high-risk pregnant women and newborns is rapidly increasing due to the declining birth rate, rising maternal age, and increase in infertility treatments. However, there is an absolute shortage of obstetrics and neonatology specialists to handle this demand. In addition, the workload and attrition of medical personnel are worsening due to the increase in critically ill patients, physicians’ avoidance of certain specialties, excessive on-call duties, and fear of medical liability. As a result, the capacity of local obstetrics and gynecology institutions is declining, with fewer deliveries and an increasing number of closures. Some institutions have even ceased delivering babies, undermining the sustainability of the local maternal and child health infrastructure. Japan, facing similar challenges of low birth rates and a shortage of perinatal personnel, has established a sustainable perinatal healthcare system through the training of midwives and specialized nurses, the establishment of multi-professional cooperative systems, stronger linkages among perinatal medical centers, and the introduction of full-time coordinator roles. This Japanese case serves as a model for overcoming the structural limitations in the medical workforce through institutional design, and provides important implications for reforming Korea's maternal and child health care system. This study analyzes Japan's policy experience and suggests policy improvements, including the training of midwives and specialized nurses, shifting the focus of support programs in areas with limited availability of obstetrical providers toward service linkage, and incorporating capable universities and specialized hospitals into the system. This approach can be utilized as a practical policy basis for complementing short-term limitations in human resource supply and demand and establishing a sustainable maternal and child health system in the mid- to long-term.

Abstract

This study examined how the work of Youth Support Managers is organized in ways that make long-term retention difficult, focusing on the short tenure and high turnover intentions of youth center workers. Using institutional ethnography, the study traced how institutional texts organize managers’ work from their standpoint. In-depth interviews were conducted with 19 Youth Support Managers and senior managers at Seoul Youth Centers and six additional informants from center management and metropolitan, district, and city-level institutions, together with the collection of relevant texts. Sequence analysis was used to trace how these texts coordinate managers’ work. The analysis identified eleven patterns of work organization across program planning, operation, and evaluation that made continued employment difficult. A performance-oriented contracting-out regime, a partially institutionalized social welfare regime, and a youth-friendly regime operated together within a single work process to coordinate managers’ work. Standards for ensuring young service users’ participation rights were concretely specified, whereas institutional conditions supporting young workers’ safety and continued employment were not sufficiently established. This study demonstrates how institutional texts and relations organize Youth Support Managers’ work in ways that make long-term retention difficult and extends the analytic focus of youth policy research from young people as policy recipients to the work of Youth Support Managers as policy implementers. It also extends institutional ethnography from social welfare and human services to a youth policy delivery system still undergoing institutionalization.
