노인장기요양보험 효과에 대한 탐색적 분석: 한국복지패널 1차부터 10차 자료를 이용하여

Exploratory Analyses on the Effects of the National Long-term Care Insurance: Using the Korea Welfare Panel Study (2006–2015)

초록

급속한 고령화와 가족구조의 변화로 노인돌봄에 대한 사회적 책임이 강조되고, 그 결과 2008년 7월부터 노인장기요양보험이 시행되었다. 노인장기요양보험법에 명시하고 있는 바처럼 장기요양보험제도의 궁극적인 목적은 삶의 질 향상이다. 본 연구의 목적은 노인장기요양 욕구가 있는 사람들을 대상으로 장기요양보험 이용이 이용자의 삶의 만족도 궤적에 영향을 주는지를 살펴보는 것이다. 구체적으로 (1)장기요양보험 ‘이용 여부’, (2)장기요양보험 ‘이용 햇수’, 및 (3)장기요양보험 ‘지속이용여부’에 따라서 삶의 만족도 궤적이 차이가 있는지를 잠재성장모형 분석을 통해 살펴본다. 자료는 한국복지패널 1차부터 10차까지를 이용하였고, 분석대상은 2006년 기준으로 노인장기요양보험제도의 법적 대상자 연령인 65세 이상 노인들 중 노인장기요양 욕구가 있다고 응답한 1,120명이다. 분석결과 (1)장기요양보험 이용여부(최소한번이라도 이용=1, 한번도 이용안함=0)의 경우 최소 한번 이상 이용한 사람들의 삶의 만족도 궤적이 이용하지 않은 사람들보다 긍정적으로 나타났지만, 통계적으로는 유의하지 않게 나타났다. (2)대상자들의 장기요양보험 이용햇수(최소 0~최대 7년)가 많을수록 삶의 만족도가 유의하게 더 빠르게 증가하는 것으로 나타났다. (3)장기요양보험을 지속적으로 이용한 사람들의 경우(7년동안 지속적 이용=1) 나머지 사람들에 비해서 삶의 만족도 증가속도가 더욱 빠른 것으로 나타났다. 이러한 결과는 전체적으로 노인장기요양보험이 대상자의 삶의 질에 긍정적으로 영향을 미치고 있다는 점과 동시에 노인장기요양보험의 효과를 극대화하기 위해서는 일시적이거나 한시적 이용보다는 수급기회를 최대화 하고 가능하면 지속적인 이용이 필요하다는 것으로 해석된다. 연구결과를 바탕으로 본 연구의 함의 및 후속연구의 필요성을 논의하였다.;Due to the rapid increase of aging population and the changes in perception of elderly care, the National Long-term Care Insurance (NLCI) has been enacted since 2008. As specified in the Law on NLCI for Senior Citizens, NLCI aims to promote people’s quality of life. Using the latent growth modeling, this study aims to examine whether (1) the experience of using NLCI (at least once = 1), (2) the total years of using NLCI (min. 0~max. 7 years), and (3) the continuous use of NLCI (7 years of NLCI use=1, all others =0) are associated with the trajectory of life satisfaction (LS) among the elderly who had long-term care needs. The Korean Welfare Panel Study Data from Waves 1 to 10 were used, including 1,120 elderly aged 65 and over who reported that they had long-term care needs as of 2006. Major findings are as follows. (1) Those who used NLCI at least once presented more positive LS trajectory but it was not statistically significant. (2) Those who used NLCI for more years presented more positive LS trajectory than their counterparts, which was statistically significant. (3) Those who used NLCI continuously for 7 years presented much more positive LS trajectory than their counterparts. The results further suggest that the effects are getting stronger when the period of using NLCI is longer and much stronger when it is used continuously. Based on the results, implications and future research were discussed.
