In the midst of a heated online controversy over fenbendazole use in cancer patients, this study aimed at understanding psychological mechanisms underlying the use of alternative medicine as well as features of the institutional environment fostering such controversies. We conducted a thematic analysis of 2,146 user comments on two most commented online news articles featuring people with diverging attitudes towards fenbendazole use. The three themes that comprised the highest proportions of these comments were “longing for a miracle” (18%), “the use of fenbendazole is understandable in the case of terminal cancer” (17%), and “distrust of professionals” (16%). The alleged evidence presented in comments supporting the use of fenbendazole in terminal cancer patients was mostly cases of improved patients (2.3%), followed by scientific evidence (0.9%) and expert opinion (0.4%). These results suggest that favorable public attitudes towards terminal cancer patients’ use of alternative medicine such as fenbendazole might stem from their desperate desire for miracle and survival and that the health care institution is a contributor to such controversies. Understanding the psychological mechanisms behind the use of alternative medicine in cancer patients has important implications for reconciling the discrepancy between health care professionals and patients in their attitudes towards alternative medicine as well as helping patients make better decisions. Aside from investing in research for evidence production, a greater societal effort is required to improve access to and understanding of medical evidence as well as to enhance the public’s trust of the medical institution.
이 연구는 암환자의 개구충제 복용을 둘러싼 논란의 시기 한가운데에서, 온라인 뉴스의 댓글을 통해 대중의 태도와 암 보완대체요법 이용에 긍정적인 태도를 낳는 심리적 기제를 이해하고자 하였다. 논란이 고조되었던 2019년 10~11월 중 댓글 수가 가장 많고 대조적인 입장을 다룬 두 온라인 뉴스에 달린 2,146개의 댓글을 대상으로 테마분석을 수행하였다. 가장 높은 비율을 차지한 세 개의 테마는 기적에 대한 열망(18%), “말기암이라면 그럴 수 있다(17%)”, 전문가 집단에 대한 불신(16%)이었다. 또한, 개구충제의 치료효과와 관련된 믿음의 바탕에는 호전된 사례, 즉 개인의 직･간접적 경험이 가장 많았고, 과학적 근거나 전문가의 의견은 가장 적었다. 암환자의 개구충제 복용은 말기암이라는 예외적이고 특수한 상황에서 기적에 대한 간절함, 죽음을 막고 싶은 절실함과 같은 심리적 기제로 이해할 수 있으며, 전문가집단에 대한 불신도 기여하는 것으로 보인다. 온라인 뉴스 댓글이라는 연구 자료의 특수성을 감안하더라도, 이러한 결과는 근거기반 의학의 이상과 대중의 태도라는 현실 사이의 현격한 차이를 드러낸다. 의학적 근거의 생산을 위한 투자 외에도, 근거에 대한 대중의 접근성 및 이해력 향상, 의료에 대한 신뢰 제고를 위해 더 많은 사회적 노력이 필요하다.
This study aimed to identify the current status and challenges of the selection pathway―a causal mechanism through which patient experience measurement and public reporting promote quality improvement in healthcare systems by influencing patients' choice of healthcare providers―and to identify key areas for improving the patient-centeredness in the healthcare system from the patient's perspective. Data were collected through focus group discussions and one-on-one in-depth interviews with patients, followed by thematic analysis. The analysis yielded twelve subthemes categorized under three main themes: i) perceptions and attitudes toward patient experience assessment, ii) comprehensive improvement for patient experience assessment survey systems, and iii) tasks for improving patient-centeredness in the healthcare system. Patients expressed strong support for the necessity and value of the Patient Experience Assessment, while also identifying areas of improvement to better activate the selection pathway. Furthermore, participants proposed long-term strategies for promoting a more patient-centered healthcare system, including comprehensive healthcare quality improvement, reforms in medical education, and systemic changes in the healthcare system. This study is significant in that it directly involved patients during the early stages of implementing the Patient Experience Assessment and contributed to understanding how effectively the selection pathway operates and identifying areas for improvement.
이 연구는 환자경험 평가와 의료체계의 환자중심성 향상을 연결하는 이론적 인과 기전 중 하나인 선택 경로의 현황과 개선 과제를 규명하고, 환자의 관점에서 의료체계의 환자중심성 향상을 위한 과제를 파악하는 것을 목적으로 하였다. 환자 대상 초점집단토의와 일대일 심층면접을 수행하여 자료를 수집하고, 주제 분석을 실시하였다. 분석 결과, '환자경험 평가에 대한 인식 및 태도', '환자경험 평가의 포괄적 개선방안', '보건의료체계의 환자중심성 향상을 위한 과제'의 세 가지 주제로 포괄되는 12개 하위 주제가 도출되었다. 환자들은 환자경험 평가의 필요성과 가치에 공감하며 긍정적인 반응을 보였으나, 선택 경로가 효과적으로 작동하기 위해서는 해결되어야 할 다양한 개선 과제가 존재하였다. 또한 향후 의료체계의 환자중심성 향상을 위해 필요한 장기적인 과제로서 포괄적인 의료의 질 향상, 의학교육 개선, 전반적 의료체계 개선 등이 제시되었다. 이 연구는 환자경험 평가 시행 초기 단계에서 직접 환자를 대상으로 선택 경로의 작동 현황을 파악하고 개선 과제를 도출하였다는 점에서 의의가 있다.
