This study is a qualitative examination of the ‘Experiences of Users and Providers of Musculoskeletal Discharge Linkage Service’, a project initiated by the Ministry of Health and Welfare to establish a public health and medical cooperation system. Data were collected through in-depth interviews with both users and providers of the musculoskeletal discharge linkage service at Seonam Hospital in Seoul, conducted from September 23 to October 14, 2024. From the experiences of the users, 11 subcategories and 3 categories were derived: ‘Factors hindering the use of discharge linkage service’, ‘Satisfactory discharge linkage service’, and ‘Most necessary resources for discharge to home.’ From the experiences of the providers, 10 subcategories and 3 categories were derived: ‘Factors hindering return to the community after discharge’, ‘Factors for success in discharge linkage service’, and ‘Support system for successful return to the community.’ This study is significant in that it comprehensively analyzes the experiences of discharge linkage services, incorporating the multifaceted perspectives of both users and providers. Notably, it highlights the differences in perception between users and providers on topics such as the need for discharge-linked services, the importance of early intervention during hospitalization, the need for standardized practices for multidisciplinary cooperation, and strategies for enhancing self-management capabilities. These findings underscore the importance of continuous health management through linkages between hospitals and local communities. The results can therefore serve as foundational data for improving the operation of discharge-linked services and for establishing business models for responsible medical institutions nationwide.
본 연구는 보건복지부 공공보건의료 협력체계 구축사업인 ‘근골격계 퇴원연계서비스 이용자 및 제공자 경험’에 관한 질적 연구이다. 2024년 9월 23일부터 10월 14일까지 서울특별시 서남병원 근골격계 퇴원연계서비스 이용자 및 제공자를 대상으로 일대일 심층면담하여 자료를 수집하였다. 퇴원연계서비스 이용자 경험에서 하위범주 11개, 범주 3개가 도출되었다. ‘퇴원연계서비스 이용방해요인’, ‘만족스러운 퇴원연계서비스’, ‘집으로 퇴원하기 위해 가장 필요한 자원’으로 도출하였다. 퇴원연계서비스 제공자 경험에서 하위범주 10개, 범주 3개가 도출되었다. ‘퇴원 후 지역사회 복귀 방해요소’, ‘퇴원연계서비스 성공요소’, ‘성공적 지역사회 복귀를 위한 지지체계’로 도출되었다. 본 연구는 이용자와 제공자의 다각적인 시각을 반영하여 퇴원연계서비스의 경험을 통합적으로 분석했다는 점에서 의의가 있다. 특히 연구를 통해 드러난 퇴원연계서비스 필요성에 대한 이용자 및 제공자 간 인식의 차이, 입원 초기 개입의 중요성, 다학제적 협력을 위한 실무 표준화의 필요성, 자가관리 역량 강화를 위한 전략은 병원과 지역사회 간 연계를 통한 지속적인 건강관리의 중요성을 강조하였다. 이에 퇴원연계서비스 실제 운영 개선뿐 아니라, 전국의 책임의료기관 대상의 사업모델 수립에 기초 자료로 활용될 수 있기를 기대한다.
This study aims to explore the potential role of community members in integrated care for older adults through an action research approach. The research was conducted in Jeongeup City, Jeollabuk-do, one of the depopulated cities in South Korea. Sixteen community volunteers were selected as "Village Care Managers" and assigned to various towns and villages. Their activities included identifying health and welfare needs of older adults, coordinating AI-based care calls and follow-up actions, organizing health promotion programs, and linking community resources. Three joint case conferences with local health and welfare organizations were held during the intervention, and the analysis focused on three cases where resource linkage was attempted. The findings revealed that resident-led case management effectively addressed care needs by strengthening social networks and communication among community members, while also improving proximity and continuity of care. These outcomes were comparable in both quantity and quality to expert-led case management. Furthermore, trust restoration among neighbors and the initiation of voluntary mutual care were notable achievements. However, challenges included limited access to health-related services, a lack of information on welfare resources, and the passive involvement of local resources in addressing complex needs, which hindered the overall effectiveness of the intervention. This study highlights the potential for community members to lead integrated care and emphasizes the need for ongoing support and research.
본 연구는 지역사회를 중심으로 지역 주민이 노인 통합돌봄 사례관리를 주도적으로 수행할 수 있는지를 검증하고자 한 실천연구이다. 초고령화가 진행된 대표적 인구감소지역인 전북특별자치도 정읍시에서 16명의 주민자원자를 선발해 “마을돌봄매니저”로 명명하고 각 읍면에 배치했다. 이들은 보건·복지 수요 확인, AI 안부전화 연결과 사후조치, 경로당 건강증진 프로그램 운영, 개별 돌봄 요구 지원, 자원연계 등의 활동을 수행하였다. 활동 기간 중 시청, 보건소, 복지관 등 지역사회 보건·복지자원과 3회의 합동 사례회의를 진행했고, 자원연계를 시도한 노인 중 3명의 사례를 중심으로 분석했다. 연구 결과, 주민주도의 사례관리는 주민 간 사회적 관계망 강화, 의사소통 증진, 근접성과 지속성을 높임으로써 효과적으로 노인의 돌봄 요구를 충족할 수 있음을 확인하였다. 이는 전문가 중심 사례관리와 양적·질적으로 유사한 성과를 보였으며, 주민 간 신뢰 회복과 자발적 상호돌봄이 촉진되는 효과도 확인되었다. 반면, 건강문제 접근의 어려움과 복지자원 정보 부족으로 복합적 요구를 가진 사례는 자원연계에 의존할 수밖에 없었고, 참여했던 지역사회 자원의 소극적 대응으로 실효성이 제한되었다. 본 연구는 지역 주민이 통합돌봄에서 주도적인 역할을 할 가능성을 제시하며, 이를 위한 체계적 장치와 실행 방안에 대한 지속적 연구의 필요성을 강조한다.
This study applied the photovoice research method to explore the daily lives of out-of-school adolescents from their own perspective and in their own voice. The study participants included seven adolescents who had been out–of–school for at least one year and had been using an out-of-school youth support organization for a minimum of one year. A total of six photovoice sessions were held. The data for analysis consisted of 211 photos submitted by the participants and the transcripts of the recorded photovoice sessions. The participants selected six topics regarding their daily lives to share their experiences. The collected data were analyzed using an inductive qualitative analysis method. The results revealed the following themes: “Choosing to leave school due to academic and health issues”, “School-free life not as easy as expected,” “Changed routines in a free and independent life,” “Balanced self-development in virtues, physical well-being, and spirituality,” “Growth through new challenges and encounters,” and “An uncertain future, yet moving forward with dreams.” This study emphasized the importance of connecting out-of-school adolescents with support systems and providing them with relevant information. It highlights the positive aspects of their growth and resilience as they strive toward college entrance and career preparation within limited out-of-school support systems.
본 연구는 학교 밖 청소년의 일상생활을 청소년 당사자의 시각과 목소리를 통해 탐색하고자 포토보이스 연구 방법을 적용하였다. 연구 참여자는 학교를 중단한 지 최소 1년 이상이며, 학교 밖 청소년 지원기관을 1년 이상 이용한 청소년 7명을 대상으로 총 6회기의 포토보이스를 진행하였다. 참여 청소년이 제출한 총 211장의 사진과 포토보이스 진행과정을 녹음한 녹취록을 분석 자료로 사용하였다. 참여 청소년들은 일상생활에 관하여 6가지의 의제를 선정하여 경험을 나누었으며 수집된 자료는 귀납적 질적분석방법을 적용하였다. 연구 결과 ‘학업과 건강 문제로 학교중단을 선택’, ‘생각보다 녹록지 않은 학교 밖 생활’, ‘자유롭고 독립적인 생활로 달라진 일상’, ‘지·덕·체·영성의 조화로운 자기계발’, ‘새로운 도전과 만남을 통한 성장’, ‘불안한 미래, 그러나 꿈을 향해 나아가는 나’의 주제가 도출되었다. 본 연구는 학교 밖 청소년과 학교 밖 지원체계와의 연계 및 정보제공의 중요성을 강조하였으며, 부족한 지지체계 속에서도 대학 진학과 진로 준비를 통해 도전하며 성장해 나간다는 긍정적인 측면을 부각하였다는 점에서 연구의 의의가 있다.
